急诊内镜下治疗食管胃底静脉曲张出血患者的应用价值

摘　　要：目的评价急诊内镜下治疗食管胃底静脉曲张出血患者的应用价值。方法在稳定生命体征情况下对50例食管胃底静脉曲张出血患者在出血12 h内进行内镜下治疗。其中30例采用食管静脉曲张套扎,6例采用胃底静脉组织胶注射,14例采用食管静脉曲张套扎联合胃底静脉组织胶注射。结果 50例食管胃底静脉曲张出血患者急诊内镜止血成功率达96.0%(48/50),2例失败;47例患者均成功进行了内镜下治疗,治疗有效率达97.9%(47/48),无效率为2.1%(1/48)。术中出血1例,8例轻度不适,未出现严重并发症。结论急诊内镜下治疗食管胃底静脉曲张出血患者能迅速止血,操作简单、实用、安全,并发症少,值得推广。Objective To evaluate the clinical efficacy of emergency endoscopic therapy for the treatment for esophagogastrie varices bleeding. Methods Using esophageal varices band ligation, gastric varices tissue glue injection, or the combination of both, fifty patients with esophageal and/or gastric varicea] bleeding were treated within 12 hours after the onset of the disease. All the endoscopic procedures were carried out after each patient＇s vitals were stablized. Results Emergency endoscopy therapy was attempted in 50 patients, with the success rate of 96.0% （48/50）. For 48 patients who were successfully treated, the therapy was effective in 97.9% of patients （47/48） and ineffective in 2.1% of patients （1/48）. Except for intraoperative bleeding in 1 case and slight discomfort in 8 cases, no severe complications were observed. Conclusion Emergency endoscopic treatment for bleeding esophageal and gastric varices is an effective, simple, practical and safe method.
