齐拉西酮与奥氮平治疗对精神分裂症患者血脂的影响

摘　　要：目的探讨齐拉西酮与奥氮平治疗对精神分裂症患者血脂水平的影响。方法将72例精神分裂症患者随机分为两组，每组36例，奥氮平组口服奥氮平治疗，齐拉西酮组口服齐拉西酮治疗，观察90d。于治疗前及治疗第15d、30d、60d、90d检测两组血脂水平，对两组检测结果进行对比分析。结果奥氮平组治疗第15d起．三酰甘油、总胆固醇水平较治疗前显著升高（P〈0．01）；第30d起，低密度脂蛋白水平较治疗前显著升高，高密度脂蛋白水平较治疗前显著下降（P〈0．01）。齐拉西酮组治疗第90d，高密度脂蛋白水平较治疗前显著升高（P〈0．01），三酰甘油、总胆固醇及低密度脂蛋白水平与治疗前比较差异无显著性（P〉0．05）。同期两组间比较，治疗第30d起，奥氮平组三酰甘油、总胆固醇、低密度脂蛋白水平显著高于齐拉西酮组，高密度脂蛋白水平水平显著低于齐拉西酮组，差异均有极昆著性（P〈0．01）。结论与奥氮平相比，齐拉西酮治疗对精神分裂症患者的血脂水平影响较小。Objective To explore effects of ziprasidone and olanzapine treatment on blood lipid of schizo- phrenia patients. Methods Seventy-two schizophrenics were randomly assigned to two groups of 36 pa tients each, olanzapine group took orally olanzapine and ziprasidone group did ziprasidone for 90 days. Blood lipid levels of both groups were detected at baseline and on day 15, 30, 60 and 90, and detection re- sults contrastively analyzed. Results On day 90 in olanzapine group triacylglycerol （TG）, cholesterol to- tal （CHO） and low density lipoprotein （LDL） levels increased more significantly and high density lipopro- tein （HDL） lowered compared with pretreatment, differences were very significant （P〈0.01） ; in ziprasi- done group HDL increased more significantly （P〈0.01） and there were no differences in TG, CHO and LDL （P〉0.05）. Since day 30, TG, CHO and LDI. were significantly higher and HDL lower in olanzapine than in ziprasidone group （P〈0.01）. Conclusion Ziprasidone has less effect on blood lipid of schizophre nia patients compared to olanzapine.
