齐拉西酮利培酮奋乃静对精神分裂症患者生活质量的影响

摘　　要：目的探讨齐拉西酮、利培酮、奋乃静对精神分裂症患者生活质量的影响，为临床治疗提供参考依据。方法将129例精神分裂症患者随机分为3组，齐拉西酮组42例，奋乃静组44例，利培酮组43例，3组均完成40例，分别口服齐拉西酮、利培酮、奋乃静治疗，观察8周。于治疗前及治疗第2周、4周、8周末采用世界卫生组织生活质量问卷评定3组患者生活质量。结果治疗前3组患者世界卫生组织生活质量问卷总分及各因子分比较差异无显著性（P〉0．05）；治疗第2周末齐拉西酮组世界卫生组织生活质量问卷总分及生理、心理、独立、社会领域评分显著升高，与治疗前和另外两组比较差异有显著性（P〈0．05）；治疗第4周末起3组患者世界卫生组织生活质量问卷总分及各因子分均显著高于治疗前（P〈0．01）；奋乃静组治疗第4周末世界卫生组织生活质量问卷总分及生理领域评分显著低于其他两组（P〈0．05），治疗第8周末生理领域评分仍显著低于齐拉西酮组（P〈O．05）。结论齐拉西酮能较快提高精神分裂症患者的生活质量，总体影响与利培酮相当；奋乃静对精神分裂症患者生活质量的改善效果较差。Objective To explore the effect of ziprasidone, risperidone and perpiaenazme on quauty o, ,le in schizophrenia patients. Methods A total of 129 schizophrenics were randomly divided into ziprasidone, risperidone and perphenazine group, they respectively took orally ziprasidone, risperidone and perphena- zine for 8 weeks. Quality of life was assessed with the World Health Organization Quality of Life Ques- tionnaire （WHOQOL-100） at baseline and at the end of the 2nd, 4th and 8th week. Results There were no significant differences in the total and each factor score among 3 groups before treatment （P〉0.05） ; since the end of the 2na week the total, physiological, psychological, independent and social field score increased more significantly in ziprasidone group compared with pretreatment and than in other groups （P〈〈0.05） ; the total and physiological field score were significantly lower in perphenazine group than in other groups at the end of the 4th week （P〈0.05）, so was physiological field score than in ziprasidone group at the end of the 8th week （P〈0.05）. Conclusion Ziprasidone could improve quality of life in schizophrenia patients more rapidly, its total effect is equivalent to risperidone; perphenazine＇s effect improving quality of life is poorer.
