摘 要:目的探讨湿性年龄相关性黄斑变性(AMD)的相干光断层成像(OCT)图像特征。方法对106例(106只眼)诊断为湿性AMD的患者进行回顾性研究。所有患者均采用Carl Zeiss OCT进行检查,采用SPSS17.0软件对OCT指标详细分析患者黄斑区结构变化。结果本组106只眼中典型性脉络膜新生血管(CNV)者47只眼,隐匿性CNV者15只眼,混合性CNV者44只眼。OCT图像示:所有患眼中出现单纯视网膜囊样水肿11只眼占总比率10.38%(其中典型型CNV 8只眼,占17.02%;隐匿性CNV 2只眼,占13.33%;混合性CNV 1只眼,占2.27%),视网膜囊样水肿合并视网膜下积液22只眼占总比率20.75%(其中典型型CNV 20只眼,占42.55%;隐匿性CNV 1只眼,占6.67%;混合性CNV 1只眼,占2.27%),视网膜弥漫性水肿合并视网膜下积液22只眼占总比率20.75%(其中典型型CNV 10只眼,占21.28%;隐匿性CNV 4只眼,占26.67%;混合性CNV 8只眼,占18.18%),色素上皮脱离合并视网膜弥漫性水肿合并视网膜下积液24只眼占总比率22.64%(其中典型型CNV 1只眼,占2.13%;隐匿性CNV 4只眼,占26.67%;混合性CNV 19只眼,占43.18%),色素上皮脱离合并视网膜囊样水肿合并视网膜下积液27只眼占总比率25.47%(其中典型型CNV 8只眼,占17.02%;隐匿性CNV 4只眼,占26.67%;混合性CNV 15只眼,占34.09%)。结论 OCT能对湿性AMD患者进行无创伤性的检查、鉴别其黄斑区视网膜组织形态改变,有着独特的、不可或缺的作用。Objective To explore the characteristics of Optical Coherence Tomography { OCT) imaging in wet age related macular degeneration. Methods A retrospective study of 106 cases ( 106 eyes } diagnosed with wet age-related maeular degeneration were conducted. All patients were examined with Carl Zeiss OCT. The structural changes of the mac- ular area, indicated by OCT parameters, were analyzed using SPSS17.0 Software. Results Among 106 cases, forty-seven patients were typical choroidal neovascularization (CNV), 15 were occult CNV, and 44 were mixed CNV. OCT images showed pure retinal eystoid edema in 11 eyes (10.38%), with 8 eyes being typical CNV, 2 eyes being occult CNV, and 1 eye being mixed CNV. Twenty-two eyes (20.75%) were found to have retinal sac-like edema and retinal effusion, a- mong which 20 eyes were typical CNV, 1 eye was occult CNV, and 1 eye was mixed CNV. In 22 eyes {20.75% ) that were found to have retinal diffuse edema and retinal effusion, 10 eyes were typical type CNV, 4 eyes were occult CNV, and 8 eyes were mixed CNV. Pigmented epithelium detachment, diffuse edema and retinal effusion occurred in 24 eyes ( 22. 64% ), among which 1 eye was typical CNV, 4 eyes were occult CNV, and 27 eyes were mixed CNV. Finally, twen- ty-seven eyes ( 25.47% ) were found to have pigmented epithelium detachment, retinal sac-like edema and retinal effusion. In those cases, 8 eyes were typical type, 4 eyes were occult CNV, and 15 eyes were mixed CNV. Conclusion OCT plays a unique and indispensable role in the non-invasive examination for patients with wet age-related macular degeneration. It is also a valuable tool to identify the macular morphology changes in these patients.
