Th17／Treg细胞平衡变化在HTLV-I相关性脊髓病／热带痉挛性截瘫患者中的价值

摘　　要：目的研究辅助性T细胞（Th17）／调节性T细胞（Treg）平衡变化在HTLV－I相关性脊髓病／热带痉挛性截瘫（HAM／TSP）疾病中的意义。方法分别应用ELISA法和流式细胞技术检测HAM／TSP患者脑脊液（CSF）上清液中的IL-17、IL-10水平及外周血Th17，Treg细胞的百分率，并计算二者的比值。结果HAM／TSP患者CSF上清IL－17含量及外周血Th17细胞百分率分别为[（4．58±0．70）pg／ml、（2．00±0．64）％]，较对照组[（0．76±0．17）pg／ml、（0．41±0．24）％]均有显著升高，差异具有统计学意义（t=5．581、6．11，P〈0．01）；HAM／TSP患者CSF上清IL-10含量及外周血Treg细胞百分率分别为[（8．64±2．22）pg／ml、（3．56±0．55）％]，较对照组[（17．79±3．08）pg／ml、（7．30±1．20）％]均有显著降低，差异具有统计学意义（t：6．368、7．49，P〈0．01），HAM／TSP和对照组外周血Th17／Treg细胞比例分别为0．55±0．10、0．06±0．03，两者比较差异具有统计学意义（t=12．11，P〈0．01）。结论Th17细胞及其效应分子IL-17在HAM／TSP患者体内均显著升高，Treg细胞及其效应分子IL-lO均较对照组显著降低，Th17／Treg细胞动态平衡的变化在HAM／TSP的发病机制中发挥一定的作用。Objective To investigate the imbalance of T helper 17 （Thl7） / CD4 ± CD25 ± regulatory cell （Treg） in HAM/TSP patients. Methods Enzyme linked immunosorbent assay （EL1SA） was performed to measure the levels of interleukin （IL-17） and IL-10 in CSF, and flow cytometry to determine the percentage of Thl7 and Treg ceils in peripheral blood of patients with HAM/TSP. Results There was a significant increase in the level of IL-17 [ （4. 58 ± 0. 70 ） pg/ml vs （0. 76 ± 0. 17 ） pg/ml, P 〈 0. 01 ] in CSF and the percentage of Thl7 cells [ （2.00 ± 0. 64） % vs （0.41 ± 0. 24） %, P 〈 0. 01 ], but a decrease in the level of IL-10 in CSF and the percentage of Treg cells in peripheral blood of patients with HAM/TSP compared with the control group. The ratio of Thl7/Treg cell （0. 55 ±0. 10 vs 0. 06 ±0. 03） in the peripheral blood of patients with HAM/TSP increased significantly compared with control group （ t = 12. 11, P 〈 0.01 ）. Conclusions The imbalance between Thl7 and Treg cells may play an important role in the pathogenesis of the disease.
