唇腭裂患儿合并其他器官畸形的种类和特点

摘　　要：目的分析不同类型唇腭裂患儿合并其他器官畸形的发生率、种类和特点。方法收集2010年7月～2011年7月笔者医院口腔科收治的885例唇腭裂患儿的临床资料,分为唇裂组、腭裂组、唇及腭裂组,对各组患儿的体格检查和超声检查结果进行统计分析。结果 885例唇腭裂患儿中共检出131例(14.8%)合并其他器官畸形,包括唇裂组18例(5.8%)、腭裂组69例(23.3%)、唇及腭裂组44例(15.8%),腭裂组患儿更易合并其他畸形(F=6.419,P<0.05)。所合并的畸形,依次分别为心脏畸形59例(44.3%)、其他颌面部畸形和骨骼肌肉畸形(15.3%)、神经系统畸形(10.7%)。结论腭裂患儿较唇裂、唇及腭裂患儿更易合并其他器官畸形,先天性心脏病是最常见合并的畸形。Objective To investigate the types and characteristics of associated mulformation in children with cheilopalatognathus. Methods From July 2010 to July 2011 , 885 children with cheilopalatognathus who presented to our hospital were enrolled in the study group, which were grouped into eheilognatbus, palatognathus and eheilopalatognathus. A number of cinical data were documented and a thorough clinical examination was conducted. Results Totally 131 （ 14.8% ） of these children were found to have other birth defects, which included 18 （5.8%） with cheilognathus, 69 （23.3 % ） with palatognathus and 44 （ 15.8% ） with cheilopalatognathus. Children with palatognathus were at a high risk of associated malformations （F = 36.16, P 〈 0.01 ）. Among these malformations, 59 （44.3%） of chil- dren were found to have congenital heart disease, which was the most common type of associated mulformation. Other orofacial defects and skeletal and muscle anomalies were the next common malformations. Disorders of the central nervous system were also frequently associat- ed. Conclusion Compared to cheilognathus and cheilopalatognathus, palatognathus has a higher risk of assoicated mulformation. Con- gential heart diseases were the most congential defects among these associated mulformation.
