摘  要:目的探讨法舒地尔对脊髓缺血再灌注损伤后血清自由基水平、受损脊髓组织中天冬氨酸特异性半胱氨酸蛋白酶3(caspase-3)表达以及下肢运动功能的影响。方法采用腹主动脉夹闭法将30只家兔制作成脊髓缺血再灌注损伤模型,并随机(掷硬币法)分为实验组和对照组。实验组腹腔内注射法舒地尔,对照组注射0.9%氯化钠溶液,共14d,建模后第1、3和5天,检测家兔血清丙二醛(MDA)、超氧化物歧化酶(SOD)、一氧化氮(NO)水平的变化;建模后第14天观察脊髓损伤的组织病理学变化,并用免疫组化染色检测受损脊髓中caspase-3的表达;建模后7、14d比较两组动物后肢运动功能。结果建模后第1、3和5天,法舒地尔组家兔血清MDA、SOD、NO水平均显著低于对照组,脊髓病理学也进一证实法舒地尔组脊髓损伤程度较对照组轻微,受损脊髓组织caspase-3的表达明显低于对照组(P<0.01);法舒地尔组家兔双下肢运动评分明显高于对照组(P<0.01)。结论法舒地尔能明显降低脊髓损伤后血清自由基水平,抑制caspase-3的表达,对缺血再灌注损伤的脊髓有明显保护作用。Objective To investigate the neuroprotective effect of fasudil on the spinal cord injured by the ischemia-reperfusion and its mechanism in rabbits. Methods The abdominal aortas were selectively ligated for 45 minutes to induce the ischemia-reperfusion injury to the spinal cord in 30 rabbits, who were randomly divided into two groups, i.e. treatment group (n=20) and control group (n=10). The rabbits in the treatment group were treated by intraperioteal injection of fasudil (10 mg/kg, once a day) and ones in the control group were treated by intraperioteal injection of physiological saline. The function of the lower limbs movement in all the rabbits was determined by Jacobs method. The serum levels of malondialdehyde (MDA), nitric oxide (NO) and superoxide dismutase (SOD) were detected by the assay kits 1, 3, 5, 7 and 14 days after the injury. The expression level of cysteine- containing aspartate-specific proteases 3 (caspase-3) in the injured spinal cord was determined by immunohistochemieal stain and the pathological changes were detected by HE stain in all the rabbits 14 days after the injury. Results The Jacobs scores in the treatment group were significantly higher than those in the control group 7 and 14 days after injury (P〈0.01). The serum levels of MAD and NO in the treatment group were significantly lower than those in the control group 1, 3 and 5 days after the injury (P〈0.01). The serum level of SOD in the treatment group was significantly lower than that in the control group 1 and 3 days after the injury (P〈0.01). The pathological changes in the spinal cord were milder, and the expression level of caspase-3 was significantly lower in the treatment group than those in the control group 14 days after the injury (P〈0.01). Conclusions Fasudil can promote the functional restoration of the lower limbs movement after the ischemia-reperfusion injury to the spinal cord in the rabbits.
