摘  要:目的:探讨健康人群中幽门螺杆菌(Hp)感染对血脂代谢和颈动脉内膜中层厚度(CIMT)的影响,并探讨其可能的发病机制。方法:检测244名健康体检者的Hp感染情况,以及血脂、血浆高敏C反应蛋白(hs-CRP)、体质量指数(BMI)等指标,并测量CIMT。对Hp阳性组和Hp阴性组的血脂和CIMT等情况进行分析。结果:共检出Hp感染者108例,未感染者136名,Hp感染率达44.3%。性别、年龄在Hp感染与非感染者间无明显差异。Hp阳性组的总胆固醇(TC)、三酰甘油(TG)、载脂蛋白B(ApoB)水平较Hp阴性组有一定程度的升高,但未发现有统计学差异(P>0.05)。而高密度脂蛋白胆固醇(HDL-C)、脂蛋白a[Lp(a)]和血浆hs-CRP水平在Hp阳性组较Hp阴性组有显著降低(P<0.01),低密度脂蛋白胆固醇(LDL-C)水平则明显升高(P=0.023),2组间BMI无明显差异。Hp阳性组CIMT较Hp阴性组明显增厚,差异有统计学意义(P<0.01)。结论:Hp感染可能通过影响脂质代谢,从而加重血管壁的炎症反应,促进动脉粥样硬化的发生、发展。Objective To study the effect of Helicobacter pylori (Hp)infection on plasma lipid and carotid-intima media thickness in healthy people and to explode its pathogenesis. Methods Status of Hp infection, lipid profile, plasma high-sensitivity C reactive protein (hs-CRP), body mass index (BMI), and carotid artery intima-media thickness(CIMT) were assessed in 244 subjects undertaken routine health examination. The lipid profile and carotid artery intima-media thickness in Hp positive and Hp negative groups were analyzed. Results Of the 244 cases,Hp infection occurred in 108 cases, the Hp infection rate was 44.26%. Hp infection had no relation with sex and age. Total cholesterol (TC), triglyceride (TG), apolipoprotein B (ApoB)were higher in Hp positive group than in Hp negative group, but not reaching statistical significance ( P〉0.05 ). While high density lipoprotein cholesterol (HDL-C), lipoprotein-a[Lp (a)], plasma hs- CRP were significantly lower in Hp positive group than in Hp negative group (P〈0.01). Low density lipoprotein cholesterol (LDL-C)was obviously higher in Hp positive group ( P=0.023 ). No significant difference in BMI was found between the two groups. CIMT in Hp positive group was significantly higher than that in Hp negative group (P〈0.01). Conclusions By influencing lipid metabolism, Hp infection might aggravate the inflammatory reaction of vessel wall and promote the development of atherosclerosis.
