黄芪纤维素强化膳对高血糖重症患者营养代谢的影响

摘　　要：目的研究黄芪纤维素强化膳对高血糖重症患者营养代谢的影响。方法将64例高血糖重症患者按随机数字表法分为研究组和对照组,每组32例。2组给予105～125kJ.kg-1.d-1的肠内营养支持,不足部分由适当肠外营养补充,共24d:研究组给予黄芪水煮液联合膳食纤维强化匀浆膳(黄芪纤维素强化膳),对照组给予复方营养混悬剂。2组肠内营养支持前后分别检测前白蛋白、白蛋白、血红蛋白、转铁蛋白、氮平衡及餐后2h血糖等代谢指标。结果研究组负氮平衡纠正,各项蛋白质代谢指标明显增高,与对照组比较差异有统计学意义(P<0.05);研究组餐后2h血糖降低,与对照组比较差异有统计学意义(P<0.01)。结论黄芪纤维素强化膳可明显改善高血糖重症患者的蛋白质代谢并降低餐后血糖,同时改善患者的营养状况。Objective To study the effect of astragalus fiber fortified diet on nutrition metabolism in critically ill patients with hyperglycemia.Methods Sixty-four critically ill patients with hyperglycemia were randomly divided into study group and control group,with 32 patients in each group.All patients received enteral nutrition support(105-125 kJ·kg-1·d-1)and appropriate parenteral nutrition supplement for 24 days.The study group was given boiled liquid of Radix Astragalis and fiber fortified homogenized diet.The control group was given compound nutrition suspension agent.The levels of prealbumin,albumin,hemoglobin,transferrin,2-hour postprandial blood glucose and nitrogen balance were measured before and after enteral nutrition support. Results Compare with control group,astragalus fiber fortified diet improved nitrogen balance,increased the levels of protein metabolism and decreased 2-hour postprandial blood glucose levels(P<0.01 or P<0.05).Conclusion Astragalus fiber fortified diet can improve protein metabolism and nutritional status and reduce postprandial blood glucose levels in critically ill patients with hyperglycemia.
