超声造影在子宫肌瘤与子宫腺肌瘤鉴别诊断中的价值

摘　　要：目的探讨超声造影在子宫肌瘤和子宫腺肌瘤中的显像特点。方法对22例疑为子宫肌层病变的患者进行超声造影,分析病灶内灌注特征,并运用TIC曲线来分析不同病变的始增时间、达峰时间及峰值强度。结果经手术及病理证实的8例子宫腺肌瘤病灶表现为多支短线样强信号自病灶周边进入内部,未见周边环状强化,达峰时间及消退时间与周边肌层差异无统计学意义。14例子宫肌瘤中18个病灶表现为瘤体周边首先增强,呈环状或半环状,然后内部快速增强,最后瘤体全部增强,达到峰值强度后,瘤体强度明显.强于子宫肌层,在消退时,瘤体中央先消退。结论子宫肌瘤和子宫腺肌瘤的超声造影灌注特点及二者始增时间、达峰时间及峰值强度对鉴别诊断有重要意义。Objective To investigate the imaging features ofadenomyosis and hysteromyoma by contrast-en-hanced ultrasound （CEUS）. Methods Twenty-two patients with suspicions myomtrium diseases were examined by CEUS. The characterizes of filling was evaluated by visual interpretation, and the start time, time to peak and peak in-tensity were analyzed by TIC. Results There were 8 patients with adenomyosis appeared as short line filling from pe-ripheral into internal, and there was no significance of time to peak and wash-out time between adenomyosis and periph- eral normal myometrium. In 14-patients with hysteromyoma, there were 18 lesions appeared as early pseudocapsule fill-ing, ring or semi-ring enhancement. Then the internal masses were enhanced, which peak intensity was higher than pe-ripheral normal myometrium. In the resolution phase, the internal masses were early degraded. Conclusion The filling pattern of hysteromyoma on CEUS was significantly different from adenomyosis. The start time, peak to time and peak intensity have important value in differential diagnosis of adenomyosis and hyseromyoma.
