盆腔肿瘤精确放疗真空袋固定技术探讨

摘　　要：目的探讨盆腔肿瘤患者精确放疗真空袋体位固定技术的优缺点,寻找改善方法。方法将48例盆腔肿瘤患者分成A、B两组,A组患者采用原有真空袋体位固定;B组采用改良加长缩窄真空袋体位固定,CT定位后在直线加速器机器上做精确放疗,应用锥形束CT(CBCT)观察重复性结果。结果两组三个方向的摆位误差比较:左右X轴方向两组差异无统计学意义(P>0.05);头脚Y轴和腹背Z轴方向均有统计学意义(P<0.01)。结论经过改良加长缩窄的真空袋体位固定技术可以明显改善盆腔肿瘤的体位固定精度,在保证治疗效果的同时可以进一步减小患者的计划靶区(PTV)范围,减少患者不良反应的发生。Objective To explore the advantages and disadvantages of vacuum bag immobilization of accurate radiotherapy in patients with pelvic tumor, and to find ways to improve the technology. Methods Forty-eight patients of pelvic tumor were divided into two groups, adopt two kinds of vacuum bag with different specifications （group A： 80 cm×65 cm; B group： 110 cm×55 cm）. Cone beam CT （CBCT） was applied to observe the results. Results The two groups showed no significant difference in X axis （P〉0.05）, but statistically significant dif- ference in Y axis and Z axis （P〈0.01）. Conclusion The immobilization of modified lengthened and narrowed vacuum bag can significantly improve the fixing precision, which can significantly reduce the planning target volume （PTV） and the adverse reactions.
