摘  要:目的探讨多巴胺受体基因与遇到吸毒和社会支持的环境因素及其交互作用对海洛因依赖者戒断后复吸的影响。方法采用一般情况调查表、自然病程问卷、社会支持评定量表,收集249例海洛因依赖者戒断后1年的一般社会人口学资料、戒断后1年遇到吸毒相关环境情况、社会支持等信息,其中复吸者(复吸组)111例,保持操守者(操守组)138例;采用TaqMan探针基因分型技术对所有复吸组和操守组的多巴胺D,受体基因(DRD2)的13个单核苷酸多态性位点进行基因分型检测;采用广义多因子降维(generalized multifactor dimensionality reduction)统计软件检测影响海洛因依赖复吸的基因一环境交互作用。结果(1)复吸组在过去1年中经常遇到吸毒相关环境的频率显著高于操守组(32%:3%;F=68.15,P〈0.01),社会支持量表得分显著低于操守组[(26.83±5.54)分:(28.99±4.44)分;t=3.41,P〈0.01];(2)DRD2基因与过去1年遇到吸毒相关环境情况及社会支持水平之间存在交互作用(检验样本准确度为0.79,P〈0.01)。结论吸毒相关环境及社会支持水平与海洛因戒断后复吸相关;DRD2基因与遇到吸毒相关环境、社会支持水平之间可能存在交互作用,并共同影响海洛因依赖戒断后1年内复吸行为的发生。Objective To understand the role of the dopamine D2 receptor gene (DRD2), encountering drug-related environmental situations and social support over the previous year for relapse in heroin dependence. Methods Information was collected from 249 heroin dependent patients at one year after abstinent by using the Natural History Interview, Social Support Rateing Scale, and other questionnaires. These questionnaires provided demographic data, drug-related environmental situations and the social support over the previous year. Thirteen tagSNPs polymorphisms on DRD2 were genotyped by TaqMan SNP genotyping assay for all participants. Generalized muhifactor dimensionality reduction (GMDR) was used to construct the gene-environment interaction model which impacting relapse in heroin dependence. Results The relapse group had higher frequencies of encountering drug-related environment (32% vs. 3% ; F = 68. 15, P 〈 0. 01 ) and less Social Support Scale scores (26. 83 ± 5.54 vs. 28.99 ± 4. 44;t = 3.41, P 〈 0.01) than that in the abstinence group in the previous year. Gene-environment interaction of GMDR analysis showed that the DRD2 gene was interact with the frequency of encountering drug-related environment and social support level to impact the relapse in heroin dependence (P 〈 0. 01 ). Conclusion The relapse of heroin dependence after treatment is related to the frequency of encountering drug-related environment, and social support. Gene-environment interaction between DRD2 gene and the frequency of encountering drug-related environment and social support may be related to heroin dependence relapse.
