摘  要:目的探讨飞行员脂肪肝与颈动脉内膜改变的相关性。方法彩超对脂肪肝组与对照组采用一点测量法获得颈动脉内-中膜厚度(IMT)值,并观察双侧颈总动脉、颈内动脉、颈外动脉及颈总动脉分叉处斑块性质及分布情况,同时做相关的血清学检测。结果超声提示脂肪肝组与对照组IMT表现出增厚的平均值和最大值及斑块发生率差异明显(P<0.001)。血液检查发现脂肪肝组的空腹血糖、胰岛素浓度、高密度脂蛋白、三酰甘油、高敏C反应蛋白、胰岛素抵抗指数及比例与对照组比较差异有高度统计学意义(P<0.001);总胆固醇、低密度脂蛋白、脂蛋白a两组水平相似;脂肪肝组谷丙转氨酶、谷草转氨酶和r-谷氨酰转肽酶水平几乎为对照组的两倍;相关分析显示,高敏C反应蛋白水平与谷丙转氨酶(r=0.38)谷草转氨酶(r=0.40)显著相关。结论超声检查颈动脉可以早期发现动脉粥样硬化及斑块,评价飞行员脂肪肝病变程度,及时评判高血脂对动脉血管弹性、血管舒缩功能的影响,是重要的监测手段。Objective To probe the correlation between fatty liver and carotid artery endarterium change in pilots. Methods Carotid artery IMT values of the fatty liver group and the control group were achieved by color doppler ultrasound one-point measurement.The plaque properties and distributions on bilateral common carotid artery, internal carotid artery, external carotid artery, and common carotid artery bifurcation were observed and conducted with related serological tests.Results Ultrasound showed there were clear differences between the two groups in terms of the average value and the maximum value of IMT and plaque occurrence rate(P 〈 0.001).Blood test showed the comparative differences between the fatty liver group and the control group were of high statistical significance (P 〈 0.001 )in terms of fasting blood glucose, insulin concentration, high-density lipoprotein, riacylglycerol, high-sensitivity C-reactive protein, insulin resistance index and rate.The levels of total cholesterol, low-density lipoprotein, and apolipoprotein a in the two groups were similar.The levels of glutamic-pyruvic transaminase, glutamic-oxalacetic transaminase, r-glutamyl transpeptidase in the fatty liver group were almost twice those in the control group.Correlation analysis showed high-sensitivity C-reactive protein was closely related to glutamic-pyruvic transami- nase (r =0.38)and glutamic-oxalacetic transaminase (r =0.40).Conclusion Carotid artery ultrasound examination can early detect atherosclerosis and plaque, assess the fatty liver lesion degree of pilots, promptly decide the influence of hyperlipidemia on arterial vascular elasticity and vasomotor function, which is an important monitoring means.
