摘 要:目的:研究调亡抑制蛋白Clusterin和增殖标记物Mcm2在大肠正常组织、大肠腺瘤和大肠癌组织中的表达情况及其与临床病理特征之间的关系。方法:采用免疫组织化学SP染色的方法,选取21例大肠正常组织、34例大肠腺瘤和70例大肠癌组织作为研究对象,分别检测Clusterin和Mcm2在大肠正常组织、大肠腺瘤和大肠癌组织中表达情况,70例大肠癌组织依据性别、年龄、肿瘤大小、分化程度、浸润程度、病理分型、淋巴结转移、远处转移和Dukes分期进行分组,并分析临床病理因素之间的关系。结果:①Clusterin和Mcm2在21例正常大肠组织中不表达,34例大肠腺瘤组织中有部分表达,表达率分别为41.18%、17.65%,70例大肠癌标本有大量表达,阳性表达率分别为67.14%、54.29%,表达量在正常大肠黏膜、大肠腺瘤和大肠癌组织中依次增高,并且其表达随肿瘤病理级别的增加而增加,三者之间差异有统计学意义(P<0.05)。②Clusterin和Mcm2蛋白表达与大肠癌患者不同性别、不同年龄段、肿块大小、浸润深度、病理学类型之间无统计学意义(P>0.05)。③Clusterin和Mcm2蛋白表达与大肠肿瘤患者不同分化程度(大肠癌的Ⅰ、Ⅱ、Ⅲ级,大肠腺瘤的轻中、重度不典型增生)、有无淋巴结转移组、远处转移组和临床Dukes分期组之间的阳性表达率高于对照组。Clusterin和Mcm2蛋白在大肠癌的高、中、低分化的表达相比差异有统计学意义(P<0.05),呈逐渐升高趋势。结论:Clusterin和Mcm2在大肠肿瘤组织中呈相关关系(r=0.264,P<0.05),二者可能在肿瘤的发生发展中发挥重要的抗凋亡和促增殖作用。依据染色的组织结构差异的比较,可以准确区分大肠正常黏膜组织、大肠腺瘤组织和大肠癌组织。Clusterin和Mcm2的表达与大肠肿瘤组织的分级、临床Dukes分期、有无淋巴结转移和远处转移显著相关,可以判断大肠癌的恶性程度高低及腺瘤恶变的Objective:To research expression differences and the relationship between the clinical and pathological features In the large intestine normal tissue,large adenomas and colorectal cancer about the apoptosis inhibiting protein Clusterin and Proliferation markers Mcm2.Method:In this experiment,21 cases large intestine normal tissue,34 cases large adenomas and 70 cases Colorectal cancer were studied by immunohistochemical S-P dyeing methods.To detect separately expression of Clusterin and Mcm2 in large intestine normal tissue,large adenomas and Colorectal cancer.70 cases of Colorectal cancer which based on sex,age,tumor size,degree of differentiation,infiltrating degree,Dukes stage,lymph node metastasis,Distant metastasis were grouped.Furthermore analysis relationship of clinical pathological factors about Clusterin and Mcm2.Results:①Clusterin and Mcm2 do not express in the 21 case normal colonic organization,Clusterin and Mcm2 are part of expression in the 34 case large adenomas organization,Positive expression rates respectively of Clusterin and Mcm2 in the 34 case large adenomas organization were 41.18%and 11.76%.All are expression in the 70 Colorectal specimen.Positive expression rates respectively of Clusterin and Mcm2 in the 70 case large adenomas organization were 67.14%and 54.29%.Quantity of expression is from low to high in the normal colonic mucosa、large adenomas and Colorectal cancer tissue.And it is rise with Tumours level.There are Significant in three median differences(P0.05).There are no significant differences among the three(P0.05).②Expressions of Clusterin and Mcm2 in colorectal cancer patients with different sex,different age,tumor size,infiltration depth,pathological type was no significant correlation(P0.05).③The expressions of Clusterin and Mcm2 in colorectal cancer patients were significantly higher in lymph node metastasis group,installment Dukes group,distance metastasis group than in their counterparts(P0.05).The expression of Clusterin and Mcm2 in colorectal
