摘  要:目的探讨术前髂内动脉导管化疗对膀胱尿路上皮癌组织中HIF-1α、Ki-67和VEGF表达的影响及临床意义,以从分子生物学角度对动脉导管化疗的疗效进行评估。方法采用免疫组化SP法对46例膀胱尿路上皮癌髂内动脉导管化疗前后组织中HIF-1α、Ki-67和VEGF进行检测,并观察化疗后癌组织超微结构的变化。结果动脉导管化疗前后HIF-1α、Ki-67和VEGF的阳性表达率分别为76.1%VS5212%、87.0%VS65.2%和60.9%VS37.0%,均具有统计学差异(P〈O.05)。化疗后癌组织病理分级明显降低,且超微结构观察到的癌细胞有明显受损改变,,结论检测HIF-1α、Ki-67和VEGF对膀胱尿路上皮癌髂内动脉导管化疗的疗效判断有重要意义,髂内动脉导管化疗能明显降低肿瘤标记物的表达,作为一种新辅助化疗方案将为临床推广应用提供理论依据,可成为以后一种可信、有效的综合治疗手段.Objective To investigate the influence of preoperative Iliac Intra-arterial chemotherapy on expression of HIF-1α、Ki-67 and VEGF in urothelial carcinoma of bladder,so as to assess the curative effect of intra-arterial chemotherapy from the level of molecular biology. Methods Expressions of HIF-1α、Ki-67 and VEGF were examined by SP immunohistochemical staining in 46 eases Urothelial Carcinoma of Bladder before and after Intra-arterial chemotherapy.Cancer tissues after Intra-arterial chemotherapy were examined ultramicroscopically. Results Positive rate of HIF-1α、Ki-67 and VEGF expression before and after Intra-arterial chemotherapy were 76.1%VS52.2%,87.0%VS65.2% and 60.9%VS37.0%,They have statistically significantly differences in the positive rate of their expressions between before and after Intra-arterial chemotherapy (P〈0.05).We found the pathological grading of cancer tissues were reduced obviously after the arterial chemotherapy,and the cancer cells were severe damaged ultramicroscopically .Conclusion Detecting the expression of HIF-1α、Ki-67 and VEGF may help us to esimate the effect of Iliac Intra-arterial Chemotherapy for bladder cancer.And preoperative Intra-arterial chemotherapy can downregulate the expression of the factors mentioned above.It will provide theoretical basis for popularization and application of clinical work as a neoadjuvant chemotherapy method and could be a dependable and efficient comprehensive trealment method later.
