牙周基础治疗对2型糖尿病伴慢性牙周炎患者血糖代谢指标及血清生化指标的影响

摘　　要：目的:全面评估2型糖尿病伴慢性牙周炎患者牙周基础治疗对牙周临床参数、血糖代谢指标及血清生化指标的影响,探讨牙周基础治疗的干预作用。方法:选择2011年4月至2012年5月来北京大学第三医院口腔科就诊的2型糖尿病伴慢性牙周炎患者40名,于基线水平和牙周基础治疗后3个月时进行牙周临床检查,以及糖化血红蛋白、空腹血糖和其他血清生化指标的检查,比较治疗前、后的差异。结果:牙周基础治疗3个月后,2型糖尿病伴慢性牙周炎患者牙周临床参数(探诊深度、附着丧失、菌斑指数、出血指数、探诊后出血百分数)均有显著改善,差异有统计学意义(P<0.001)。治疗后糖化血红蛋白水平降低,差异有统计学意义(P=0.014),但空腹血糖浓度改变,差异无统计学意义(P=0.586);其他有统计学意义改变的血清生化指标包括:血清磷显著升高、血清钙磷比显著下降、甘油三酯显著下降、载脂蛋白A1显著升高、脂蛋白a显著降低和血清总胆红素显著升高。结论:牙周基础治疗对2型糖尿病伴慢性牙周炎患者牙周临床参数和糖化血红蛋白的控制有显著改善作用,但对空腹血糖的作用不明显。此外,牙周基础治疗还可能影响血清磷的浓度及钙磷比,并影响血脂的代谢。Objective：To investigate the effect of periodontal therapy on periodontal clinical index, metabolic control and serum biochemical markers in subjects with type 2 diabetes and chronic periodonti- tis comprehensively. Methods： Forty patients with type 2 diabetes and chronic periodontitis were en- rolled, and periodontal clinical parameters, HbAlc, fasting blood glucose （FBG） and serum biochemical markers were tested at baseline and 3 months after initial periodontal therapy. Results： The periodontal clinical parameters （ PLI, BI, PD, CAL and BOP） improved significantly 3 months after initial periodontal therapy （P 〈0.001）. The HbAle also decreased statistically （P =0.014）, while there was no change in venous fasting blood glucose. Other serum biochemical markers changed significantly including serum phosphor increased, calcium-phosphorus ratio decreased, triglyceride decreased, Apo A1 increased, lipoprotein a decreased, and serum total bilirubin increased. Conclusion： The initial periodontal therapy can significantly improve periodontal clinical parameters and HbAle control, but the effect on venous fasting blood glucose was not revealed yet. In addition, initial periodontal therapy can also intervene ser- um phosphorus, calcium-phosphorus ratio, and the metabolism of lipid.
