摘 要:目的探讨周围型小肺癌的CT表现,提高早期诊断效能。方法回顾性分析100例经手术病理证实的周围型小肺癌的影像学表现,对各种CT征象与病灶形态的关系进行统计对照。结果周围型小肺癌形态上分为3型,实性高密度结节68例,磨玻璃密度结节20例,中心部高密度实性结节周围磨玻璃密度结节(混合型)12例。分叶征72%,血管穿过征67%,毛刺征58%,空泡及支气管气相33%,胸膜凹陷征27%,血管集束征19%,晕征12%。3种形态肺癌中,分叶征、血管穿过征、毛刺征、胸膜凹陷征具有统计学意义(P〈0.05),空泡征及支气管气相、晕征具有显著统计学差异(P〈0.01),血管集束征无统计学差异(P〉0.05)。结论分叶征、血管穿过征、毛刺征是周围型小肺癌的基本征象,各种形态小肺癌的CT征象有一定的特征。Objective To investigate CT features of small peripheral lung cancer. Methods The CT features of 100 small periph- eral lung cancer patients who were pathologically confirmed were analysed retrospectivly. Their imaging features and lesion morphol- ogy were statistically compared. Results Small peripheral lung cancer was divided into three types according to CT findings. 68 ca- ses were solid high-density nodules, 20 cases were ground-glass density nodules, 12 cases were high-density solid nodules of center part and the surrounding ground-glass density nodules (hybrid). 72 % patients with sublobe sign, 67 % patients with vessels through the tumor sign, 58 % patients with spiculation,33 % patients with vacuoles and air brouchogram, 27 % patients with pleural indenta- tion sign, 19% patients with vascular convergence sign, and 12% patients with halo sign. Among the three types of lung cancer, sublobe sign, vessels through the tumor sign, spiculation sign and pleural indentation sign were statistical difference(P〈0.05). Vacuole sign, air brouchogram, and halo sign were statistically significant difference(P〈0.01). Vascular convergence Sign was no statistical difference(P〉0.05). Conclusion Sublobe sign, blood vessels through the tumor sign, spiculation are the basic and char- acteristic CT findings of small peripheral lung cancer.
