淋巴管密度在子宫内膜癌淋巴结转移中的作用

摘　　要：[目的]检测月经周期正常子宫内膜层和肌层以及子宫内膜癌组织中淋巴管密度(LVD),分析其与子宫内膜癌淋巴结转移的相关性。[方法]收集正常子宫全层标本30例(15例增殖期,15例分泌期),子宫内膜癌标本40例(20例Ⅰa期,20例Ⅲc期),应用D2-40抗体进行免疫组化染色,显微镜下计数LVD。[结果]在正常月经周期的子宫内膜层和肌层均有D2-40阳性染色的淋巴管,淋巴管与螺旋小动脉伴行。正常肌层LVD显著高于内膜层(10.70±1.16/mm2vs4.98±0.84/mm2;P<0.01);癌灶内部LVD(Ⅰa期3.49±0.94/mm2,Ⅲc期2.43±1.08/mm2)较正常内膜层和肌层均有所降低,差异显著(P<0.01)。而癌灶周边LVD升高,高于正常内膜但是低于正常肌层(P<0.05)。有淋巴结转移组癌灶周边LVD(8.63±2.88/mm2)较未转移组显著增高(7.09±1.22/mm2,P=0.012)。[结论]癌灶周边淋巴管与子宫内膜癌的淋巴结转移发生有关。[Purpose] To investigate lymphatic vessel density（LVD） in normal endometrial and nor- mal myometrial tissues during normal menstrual cycle and in endometrial carcinoma tissue, and to analysis its correlation with lymphatic metastasis of endometrial carcinoma. [Methods] Full thickness uterine samples （15 cases with proliferative period; 15 cases with secretory period） and endometrial carcinoma samples （20 cases with stage I a; 20 cases with stage HI c） were collected to detected LVD microscopically using D2-40 mAb immunohistochemically. [Results] Lymphatic vessels with D2-40 positive staining were located in normal endometrium and normal myometrium of menstrual cycle, and accompanied with spiral arterioles. LVD of the normal myometrium was significantly higher than that of the normal endometrium （10.70±1.16/mm2 vs 4.98＋0.84/mm2, P〈 0.01）. Intratumoral LVD was significantly decreased in both stages of endometrial carcinoma （stage I a 3.49＋0.94/mm2, stage HI c 2.43±1.08/mm2） when compared with normal endometrium and myometrium （P〈0.01）. Peritumoural LVD was significantly increased compared with normal en- dometrial LVD but decreased compared with normal myometrial LVD （P〈0.05）. Peritumoral LVD in lymphatic metastasis group was significantly higher than that in non-lymphatic metastasis group （8.63 ± 2.88/mm2 vs 7.09 ±1.22/mm2, P=0.012）. [ Conclusion ] Peritumoral lymphatic vessels corre- late to lymphatic metastasis of endometrial carcinoma.
