摘 要:目的探讨瘦素及瘦素受体与食管鳞状细胞癌的关系,为食管癌的防治提供依据。方法收集32例食管鳞状细胞癌患者资料,以20名健康人为对照,采用酶联免疫吸附法(ELSIA)和免疫组织化学染色法分别测定食管鳞状细胞癌组织及正常组织中瘦素及其受体的表达情况,分析其与临床病理因素的关系。结果食管鳞状细胞癌组织及正常食管组织中瘦素水平分别为(4.7±1.9)ng/ml和(2.54±1.01)ng/ml,差异有统计学意义(t=4.64,P〈0.001)。瘦素受体的表达率分别为81.25%(26/32)、75.00%(15/20),差异无统计学意义(χ2=0.012,P=0.91)。瘦素的表达与食管鳞状细胞癌的发生部位、淋巴结转移有关(P〈0.05),与组织学、TNM分期无关(P〉O.05)。结论食管组织中瘦素水平的升高可能是食管鳞状细胞癌发生的一个危险因子,瘦素受体的持续性表达可能为瘦素的信号转导提供基础。Objective To explore the relationship of leptin, leptin receptor and esophageal squamous cell carcinoma, and provide a scientific basis for prevention and treatment of esophageal cancer. Methods Samples were collected from 32 patients with esophageal squamous cell carcinoma, 20 healthy control subjects in Shanxi Tumor Hospital. ELISA and immunohistochemistry were used to analyze leptin and leptin receptor, respectively. The correlation between their expression and clinicopathologieal parameters was also analysized. Results Patients with esophageal squamous cell carcinoma had significantly (t = 4.64, P 〈 0.001) higher leptin levels [(4.7±1.9) ng/ml] compared with normal [(2.54 ±1.01) ng/ml] oesophagus tissues. The expression rate of leptin receptor in esophageal carcinoma and normal esophagus was 81.25 % and 75.00 % respectively,not differ significantly. The expression levels of leptin was associated with position, lymphatic metastasis. Conclusion Higher leptin levels seem to represent an additional, independent risk factor for esophageal squamous cell carcinoma, leptin receptor expression on oesophageal epithelial cells provides a pathway for leptin-mediated signal transduction.
