摘 要:目的 探讨经组织学证实的 11例乳腺浸润性小叶癌 (ILC)的细胞学穿刺涂片 ,能否进行正确的组织学分类。方法 收集 11例经组织学证实的ILC的细胞学穿刺涂片 ,同时与乳腺浸润性导管癌FNAC进行对比观察。并应用图像分析系统行细胞形态分析。结果 发现ILC在细胞学上具有一定的特点。其细胞体积小 ,核及核仁亦小 ,异型性不明显。涂片中细胞数量较少 ,单个散在的癌细胞较多 ,以小细胞群为多。约半数以上病例表现为单个小细胞呈线样或“列兵样”排列。结论 以上特点反映了的组织学结构。我们认为针吸细胞学对于多数ILC病例可以作出乳腺浸润性小叶癌的分型诊断。Objective To approach whether cytology may accurately subclassify invasive lobular carcinoma (ILC) of the breast. Methods Fine needle aspiration cytology(FNAC) specimens from 11 cases of histologically proved invasive lobular carcinoma were studied and compared with 11 cases of invasive ductal carcinoma (IDC). Imaging analysis system was used to evaluate the cell morphology. Results The cytologic characteristics of ILC were as follows: 1. The cancer cells of ILC were small, uniform with round nuclei and inconspicuous nucleoli. 2. In the smear, the cells were relatively few, single isolated and more in number, especially in the small cell group. 3. In more than half of all the cases, cancer cells were arranged in linear or “Indian file” pattern. Conclusion The above cytological characteristics may represent the histological features of ILC, and the subclassification of ILC can be made with cytological smears in most of cases.
