摘　　要：目的检测乳腺癌组织中α-SMA和MMP-9的表达情况,探讨其与乳腺癌侵袭转移的关系。方法应用免疫组化通用型二步法检测58例乳腺癌中α-SMA和MMP-9的表达,并分析两者的相关性,探讨其与乳腺癌临床病理特征的关系。结果①10例正常乳腺组织中,间质纤维母细胞α-SMA(-);58例乳腺导管原位癌、导管原位癌微灶浸润和浸润性导管癌间质纤维母细胞α-SMA表达呈不同程度上调;浸润性导管癌间质纤维母细胞α-SMA表达与肿瘤大小和组织学分级无关(P>0.05),而与淋巴结转移和临床分期有关(P<0.05)。②正常乳腺组织MMP-9阳性率为20%(2/10),而乳腺浸润性导管癌中阳性率为70.7%(29/41),明显高于正常组织(P<0.01);MMP-9阳性表达与淋巴结转移、临床分期和肿瘤大小有关(P<0.05),而与乳腺癌组织学分级无关(P>0.05)。③α-SMA在乳腺浸润性导管癌相关纤维母细胞中的表达与癌细胞MMP-9的表达呈正相关(P<0.01)。结论α-SMA阳性的乳腺癌相关纤维母细胞可能参与了MMP-9降解基底膜和细胞外基质的过程,与乳腺癌侵袭转移密切相关。Objective To study the expression of α-SMA and MMP-9 in breast cancer and their relationship with tumor invasion. Methods We used immunohistochemical technique to detect the expression of α-SMA and MMP-9 in 58 cases of breast cancer, and analyzed its relationship with clinicopathological features. Results The expression of α-SMA in breast cancer-associated fibroblasts was upregulated as compared with normal tissue. Although the expression of α-SMA had no obvious relationship with tumor size and histological grade, it was associated with the clinical stage and lymph node metastases （P 〈 0. 05 ）. The expression of MMP-9 in breast cancer cells was upregulated compared with normal tissue （P 〈 0. 05）. The expression of MMP-9 was associated with the clinical stage, lymph node metastases and tumor size （P 〈 0.05） , but had no obvious relationship with histological grade （ P 〉 0.05 ）. In addition, the expression of α-SMA in cancer-associated fibroblasts was correlated with the expression of MMP-9 in cancer cells. Conclusion The α-SMA- positive breast cancer-associated fibroblasts probably participate in the degradation of basal membrane and extracellular matrix.
