超声乳化术对高度近视合并年龄相关性白内障患者泪膜稳定性及泪液蛋白的影响

摘　　要：目的探讨超声乳化术对高度近视合并年龄相关性白内障患者泪膜稳定性及泪液蛋白的影响。方法回顾性分析我院2009年1月至2010年12月行超声乳化术的高度近视合并年龄相关性白内障患者48例(48眼),设为A组;另选择同期单纯年龄相关性白内障患者45例(52眼),设为B组,2组患者均行超声乳化术,分别于术前及术后7d、14d、30d、90d行泪膜破裂时间、基础泪液分泌试验、角膜荧光素染色检查和干眼症状评分,并对结果进行比较分析。结果与术前相比,术后7d、14d,2组泪膜破裂时间均显著下降,基础泪液分泌试验值和荧光素染色评分均显著增加,差异均有显著统计学意义(均为P<0.01);术后30d,A组泪膜破裂时间仍显著下降,差异有显著统计学意义(P<0.01),2组其他指标均逐渐接近术前水平,差异均无统计学意义(均为P>0.05);术后90d,2组泪膜指标接近术前,差异均无统计学意义(均为P>0.05)。与术前干眼症状评分相比,A组术后7d、14d、30d差异均有显著统计学意义(均为P<0.01),术后90d差异无统计学意义(P>0.05);B组术后7d差异有显著统计学意义(P<0.01),术后14d差异有统计学意义(P<0.05),术后30d、90d差异均无统计学意义(均为P>0.05)。A组和B组泪液总蛋白含量分别为(12.26±3.82)g·L(-1)、(13.54±2.51)g·L(-1),溶菌酶含量分别为(1.28±1.12)g·L(-1)、(1.35±2.23)g·L(-1),乳铁蛋白含量分别为(1.86±0.52)g·L(-1)、(1.74±0.23)g·L(-1),2组间差异均无统计学意义(均为P>0.05)。结论高度近视合并年龄相关性白内障患者泪膜稳定性较差,超声乳化术对其有明显影响。Objective To discuss the effects of phacoemulsification on tear-film stability and protein in patients with high myopia combined with age-related cataract. Methods Forty-eight patients（48 eyes） with high myopia combined with age-related cataract from January 2009 to December 2010 in our hospital were retrospectively ana- lyzed and set as group A. Another 45 patients（52 eyes） single with age-related cataract in the same time were chosen and set as group B. All patients in two groups received the phacoemulsification. The break-up time （ BUT）, Schirmer I test （ Sit）, corneal fluo- rescein（FL） staining and dry eye symptom scoring at pre-operation,7 days, 14 days,30 days,90 days after operation were examined, and the results were analyzed and com- pared. Results Compared with pre-operation,the postoperative BUT at 7 days and 14 days in two groups all obviously decreased, the SI t value and FL score all increased, there were significant differences（ all P 〈 0.01 ）. At postoperative 30 days, BUT in group A still decreased, there was significant difference （P 〈 0.01 ）, other indexes in two groupgradually reached to preoperative level, there was no statistical difference （ all P 〉 0.05 ）. At postoperative 90 days, the inde- xes all reached to preoperative level,there was no statistical difference（ all P 〉 0.05 ）. Compared with pre-operation, there were significant differences in dry eye symptom scoring at postoperative 7 days, 14 days and 30 days in group A （ all P 〈 0.01 ）, but no difference was found at postoperative 90 days （P 〉 0.05 ） ;There was significant difference at postoperative 7 days in group B （P 〈 0.01 ）, statistical difference at postoperative 14 days （P 〈 0.05 ）, no statistical difference at postoperative 30 days and 90 days （ all P 〉 0.05 ）. The total protein in tear in group A and B were （ 12.25 ± 3.82） g · L^-1 and （ 13.54 ±2.5 1 ） g · L^-1, respectively,the lysozyme levels were （1.28 ±1.12 ）g · L^-1 and （1.35
