摘　　要：目的 :探讨对不能手术的晚期肺癌进行选择性支气管动脉灌注抗癌药并评价疗效。方法 :1986年 6月至 1999年 6月对 88例不能手术的晚期肺癌 ,经皮股动脉穿刺选择性支气管动脉插管灌注抗癌药 (顺铂 CPDD+环磷酰胺 CTX或 5—氟脲嘧啶或丝裂霉素 MMC)并与随机抽出同期单纯静脉化疗 10 0例晚期肺部癌作比较。结果 :1年生存率为 6 9.3% (6 1/ 88) ,2年生存率为 54.5% (4 8/88) ,3年生存率 15.9% (14/ 88) ,5年生存率为 9.1% (8/ 88)。静脉化疗对照组 5年生存率低于 3%。(2例灌注加手术的病例有 3例术后生存 5年以上 ,5年生存率为 2 5%。结论 :支气管动脉插管化疗的CR+PR率与单纯静脉化疗的各组比较 P均 <0 .0 1,提示支气管动脉插管化疗在延长晚期肺癌寿命及改善其生存质量方面明显优于单纯静脉化疗 ,并可使部分患者获得手术机会。Objective:[WT5BZ][ST5BZ]Research the inoperable advanced lung cancer perfused anticarcinogen Via selective bronchial artery and evaluate the effect of treatment.[WT5HZ]Methods:[WT5BZ]Collecting 88 cases of inoperable advanced lung cancer,from June 1986 to June 1996 which were catheterized percutaneons transformoral artery and perfused anticarcinogen in selective bronchial artery(CPDD+CTX or 5 Fu or MMC).It is compared with 100 cases of advanced lung cancer contemporariy which were single out at random and give anticarcinogen intravenous.[WT5HZ]Result:[WT5BZ]Survival rate of one year for 69.3%(61/88),two years for 54.5%(48/88),three years for 15.9%(14/88),five years for 9.1%(8/88).Survival rate of five years in control group given anticareinogen in ravenous was less than 3%.Three of twelve cases which accepted operation and perfusion survived more than five years.Survival rate of five years is 25%.[WT5HZ]Conclusion:[WT5BZ]Survival rate of the group which were perfused anticarcinogen(CR-PR) compared with that of all groups give anticarcinogen intravenous.P<0.01. It is suggested that chemotherapy via catheterization of bronchial artery is superior to intravenous chemotherapy in prolonging life improving quality of life and makes a part of patients acquire the chance of operation.
