摘　　要：目的分析锥体束Wallerian变性的MR表现,以期提高临床诊断的准确性。方法回顾性分析37例锥体束Wallerian变性的MR表现。结果 37例中脑梗死19例,脑出血11例,脑外伤手术后4例,脑肿瘤治疗后2例,脑白质脱髓鞘1例。MR表现为大脑半球陈旧性病变,同侧放射冠、内囊后肢、大脑脚及脑桥基底部T2WI高信号区,23例病变同侧桥脑及大脑脚均明显萎缩,弥散张量成像病变侧皮质脊髓束FA明显下降。结论 MR是诊断锥体束Wallerian变性的可靠方法。Objective To analyze the MR findings of Wallerian degeneration in pyramidal tract, so as to improve the diagnosis accuracy. Methods MR findings of 37 cases with Wallerian degeneration in pyramidal tract were analyzed. Results A total of 37 cases with Wallerian degeneration in pyramidal tract including cerebral infarction in 19cerebral hemorrhage in 11, Traumatic brain injury after operation in 4, brain tumor after treatment in 2 and cerebral white matter demyelination. MR findings hyperintensity regions on T2WI were demonstrated in the ipsilateral corona radiata-posterior limb of internal capsule. The atrophy of the ipsilateral brainstem was demonstrated in 23 cases. The FA in Wallerian degeneration was significantly different compared to the normal side. Conchsion MR is valuable in the diagnosia of Wallerian degeneration in pyramidal tract.
