含呋喃唑酮四联疗法治疗消化性溃疡的临床观察

摘　　要：目的:探讨泮托拉唑、阿莫西林、呋喃唑酮和枸橼酸铋钾四联治疗幽门螺杆菌(Hp)阳性消化性溃疡的疗效及安全性。方法:75例Hp阳性的消化性溃疡患者,随机分为两组,治疗组:口服泮托拉唑40mg、阿莫西林1.0 g、呋喃唑酮0.1 g、枸橼酸铋钾220 mg;对照组:泮托拉唑40mg、阿莫西林1.0 g、克拉霉素0.5 g;两组均为每天2次,疗程7天。1周后继续服用泮托拉唑40mg,每天1次,连服3周。患者在停药后4周后行胃镜检查和快速尿素酶试验,观察溃疡愈合情况和Hp根除率及副作用的发生率。结果:两组症状缓解率分别为94.74%和94.59%,溃疡愈合率分别为94.74%和94.59%,两组比较差异无统计学意义(P>0.05),Hp根除率分别为92.11%和72.97%,差异有统计学意义(P<0.05)。结论:含呋喃唑酮的四联疗法是一种高效安全的治疗消化性溃疡和根除Hp感染的方案,不良反应症状轻微,能耐受,且价格低廉,值得推广。Objective：To explore the safety and efficacy of four couplets treatment of pantoprazole, amoxicillin, furazolidone and bismuth potassium citrate on helicobacter pylori （Hp） positive peptic ulcer. Methods：75 cases of patients with Hp positive peptic ulcer were randomly divided into two groups： treatment group： oral pantoprazole （40 mg）, amoxicillin （1.0 g）, furazolidone （0.1 g）, bismuth potassium citrate （220 mg）; The control group： oral pantoprazole （40 mg）, amoxicillin （1.0 g）, clarithromycin 0.5 g; Both groups for 2 times daily, treatment of 7 days. A week later continued to take pantoprazole （40 mg）, one time a day, served three weeks. In 4 weeks after drug withdrawal, patients underwent endoscopy and rapid urease test to observe the ulcer healing rate and H. pylori eradication rates and side effects. Results：two groups of symptoms rate were 94.74% and 94.59% respectively, ulcer healing rate were 94.74% and 94.59%, two groups of comparisons difference was no statistically significant （P 〉 0.05）, Hp eradication rates were 92.11% and 72.97% furazolidone therapy , the difference was statistically significant （ P 〈 0.05）. Conclusion：Four couplets treatment of is a kind of highly efficient and safe treatment of peptic ulcer and eradication of HP infection scheme, with few adverse reactions, mild symptoms, strong tolerance and tlow price , which is worthy to be popularized.
