男性长期饮酒者胃黏膜病变与表皮生长因子和前列腺素E2的关系

摘　　要：目的探讨男性长期饮酒者胃黏膜病变与胃液及血液中表皮生长因子(EGF)及前列腺素E2(PGE2)之间的关系。方法 2007年1月-2010年12月对符合条件的男性长期饮酒者59例进行内镜下胃黏膜活检组织的病理学观察,同时用ELISA法检测静脉血及胃液EGF及PGE2浓度。结果①长期饮酒者中重度胃黏膜慢性炎症组、中重度胃黏膜萎缩组和中重度胃黏膜肠化组患者血清EGF浓度明显高于长期饮酒者对应轻度胃黏膜病变各组患者血清EGF浓度,P均=0.000。②长期饮酒者胃黏膜不典型增生组患者血清和胃液EGF浓度明显高于长期饮酒组中胃黏膜无不典型增生组患者血清和胃液EGF浓度,P=0.000或0.022。③两组患者之间血清和胃液PGE2对比,P均>0.05。结论长期饮酒者血液EGF水平与胃黏膜慢性炎症及癌相关病变有关。胃液EGF水平仅与不典型增生发生有关。血液和胃液PGE2与胃黏膜慢性炎症、萎缩、肠化的严重程度和不典型增生是否存在无关。[ Objective ] To explore the correlation between pathological changes of gastric mucosa and epidermal growth factors （EGF） and Prostaglandin E2 （PGE2） for chronic ethanol ingestion in man. [Methods] A study of gastric mucosa acquired by gastroscope was conducted in 59 patients of chronic ethanol ingestion in man from January 2007 to December 2010. And EGF and PGE2 in venous blood and gastric juice sample were taken and examined by enzymelinked immunosorbent assay. [ Results ] ①Compared with the slight chronic inflammation group, the slight chronic atrophy group and the slight chronic intestinal metaplasia group, the concentration of EGF in serum in the moderate and severe chronic inflammation group, the moderate and severe atrophy group and the moderate and severe intestinal metaplasia group were significantly higher （all of the P =0.000）.②In contrast to the no dysplasia group in gastric mucosa, the EGF concentration in gastric juice in dysplasia group was significantly elevated. ③The concentration of PGE2 in serum and gastric juice have not significant statistic differences （P 〉0.05 in whole）. [ Conclusion] There are some significantly correlations between EGF level in serum and chronic inflammation as well as Gastric Cancer-related lesion. The EGF level in gastric juice is only related to dysplasia in gastric mucosa. But PGE2 in the serum and gastric juice had nocorrelation with the seriousness in gastric mucosa.
