腹腔镜手术治疗早期胃癌66例临床分析

摘　　要：目的探讨早期胃癌行腹腔镜手术治疗与传统开腹手术治疗的临床效果。方法对30例早期胃癌腔镜手术患者(腹腔镜组)和同期36例早期胃癌开腹手术患者(开腹组)的资料进行回顾性分析,比较两组手术质量相关指标。结果两组患者手术均顺利完成,无术中并发症发生。39例患者(腹腔镜组21例,开腹组18例)随访3～21个月,随访死亡6例。腹腔镜组术中出血量、术后离床活动、术后肠道功能恢复时间、术后离院时间等方面均优于开腹组(P<0.01),但腹腔镜组所需手术时间长于开腹组,差异有统计学意义(P<0.05)。结论腹腔镜手术治疗早期胃癌安全有效,明显优于开放手术,早期疗效满意,远期结果有待进一步研究。Objective The aim of this study is to compare laparoscopic gastrectomy and conventional open resection in the treatment of early gastric cancer. Methods Sixty-six patients with early gastric cancer were involved in the study. Thirty of the patients were treated with laparoscopic gastrectomy and 36 patients with conventional open resection during the period from 2007 to 2010 in our hospital. Intra-operative blood loss, postoperative ambulation, recovery time of postoperative intestinal function and the time of hospital stay were compared between the two treatment methods. Results The operations of all patients were success. No operation complication occurred. After leaving hospital,39 patients （21 from the laparoscopie operation group and 18 from the open operation group）were fol- lowed-up. The follow-up period was 3 -31 months. During the period of follow-up ,6 cases died. The prognosis of the laparoscopic gas- trectomy group was better than that of the conventional surgery group （P 〈 0. 01 ）. However, operation time of the laparoscopic operation group was longer than that of the open operation group （ P 〈 0. 05 ）. Condusions Laparoscopie gastreetomy is a safe technical alterna- tive to conventional open resection for early gastric cancer. It can enhance postoperative recovery. However,long term effects of laparo- scopic gastrectomy need further studies.
