摘 要:目的:提高甲状腺乳头状癌的细针针吸细胞学诊断准确性。方法:回顾分析156例组织学证实的甲状腺乳头状癌细针针吸细胞学标本,总结8项细胞学特征:排列结构、细胞数量、核内胞浆包涵体、核沟、核仁、毛玻璃样核、多核巨细胞及砂粒体。结果:156例甲状腺乳头状癌的主要细胞学特点是:①乳头状结构;②核内胞浆包涵体;③核沟;④细胞数量多;⑤大或小核仁;⑥毛玻璃样核;⑦多核巨细胞;⑧砂粒体。结论:针吸细胞学诊断甲状腺乳头状癌的主要依据是:乳头状结构、核内胞浆包涵体、核沟、细胞数量多、大或小核仁、毛玻璃样、多核巨细胞和砂粒体。针吸细胞学可以对甲状腺乳头状癌作出准确诊断。Objective: To improve the accuracy of cytologic diagnosis of papillary thyroid carcinomas (PTC) . Methods: Fine needle aspiration of 156 cases of histologically confirmed PTC was retrospectively analyzed. Eight cytologic features including cell arrangement , cellularity, intranuclear cytoplasmic inclusion body, nuclear groove, nucleoli,ground glass nuclear, multi-nuclear giant cell and psammoma bodies were reviewed.Results: The main cytologic features of the 156 PTC cases were:①papillary structure, ②intranuelear cytoplasmic inclusion body, ③ nuclear groove, ④more eellularity,⑤big or small nucleoli,⑥ground glass nuclear,⑦multi-nuelear giant cell and, ⑧psammoma bodies.Conclusion" Papillary structure, intranuclear cytoplasmic inclusion body, nuclear groove, more cellularity, big or small nucleoli, ground glass nuclear, multi-nuclear giant cell and psammoma bodies are the important evidence for papillary carcinoma of thyroid.Fine needle aspiration cytopathology may diagnose PTC precisely.
