晶状体玻璃体切除治疗外伤性晶状体脱位继发青光眼的临床观察

摘　　要：目的：观察晶状体玻璃体切除治疗外伤性晶状体脱位继发青光眼的临床效果。方法：对48例（49只眼）外伤性晶状体脱位继发青光眼患者进行晶状体、玻璃体切除手术治疗，并对术眼致伤原因、术前、后视力、眼压及术后并发症进行分析。结果：术后44例患者（45只眼）眼压〈21mmHg（91．67％）；2例患者（2只眼）眼压21-30mmHg（4．16％）；2例患者（2只眼）眼压〉30mmHg（4．16％）。35例患者术后矫正视力提高；12例患者术后矫正视力无提高；1例患者术后矫正视力较术前差。2例患者术后视网膜脱离．3例患者出现葡萄膜炎，2例患者术后眼压仍大于30mmHg，1例患者术后出现黄斑水肿。结论：继发性青光眼是外伤性晶状体脱位的最常见并发症，其发生机制与玻璃体产生的阻滞有关，晶状体玻璃体切除手术是治疗此类青光眼的一种安全有效的方法。Objective：To observe the clinical effect of lentectomy and vitrectomy on treatment of secondary glaucoma of traumatic dislocation of lens. Methods： In 48 cases （49 eyes） of traumatic dislocation of lens and secondary glaucoma treated with lentectomy and vitrectomy the operation,causes of postoperative ocular injury, preoperative visual acuity, intraocular pressure, and postoperative complications were analyzed.Results：44 cases of postoperative intraocular pressure were less than 21mmHg （91.67%）,2 cases from 21mmHg to 30mmHg,and 2 cases was more than 30mmHg.Corrected visual acuity of 35eases after operation iInproved.12 cases without improvment.and 1 case dropped. After operation, the complications included 2 retina detachment, 3 uveitis, 2 mtraocluar pressure more than 30 mmHg,and 1 macular edema.Conclusion： Secondary glaucoma is the most common complication of traumatic dislocation of lens,and its mechanism may be vitreous blocking up.Lentectomy and vitreetomy is the safe and effective methods for secondary glaucoma.
