脂联素瘦素及胰岛素抵抗与非酒精性脂肪肝的关系

摘　　要：目的:探讨脂联素、瘦素及胰岛素抵抗与非酒精性脂肪肝的关系,以了解NAFLD发病危险因素及发病机理。方法:选择病例120例,非酒精性脂肪肝患者60例,酒精性脂肪肝患者60例,另择60例健康人作为正常对照,比较各组之间的指标差异。结果:非酒精性脂肪肝组与酒精性脂肪肝组的脂联素、瘦素、内脂素、抵抗素指标的水平均高于正常对照组,具有显著性差异,P<0.05。单纯性脂肪肝亚组的脂联素水平与脂肪性肝炎亚组差异具有显著性,P<0.05。脂联素、瘦素、内脂素分别与抵抗素、HOMA-IR具有相关性,P<0.05。结论:脂联素、瘦素及胰岛素抵抗水平在非酒精性脂肪肝中均有不同程度的变化,参与了非酒精性脂肪肝的发生和发展。Objective： To investigate relationship between adiponectin, leptin and insulin resistance with non-alcoholic fatty liver disease, to understand the NAFLD risk factors and pathogenesis. Method： Select 120 cases, Non-alcoholic fatty liver disease patients 60 cases, alcoholic fatty liver disease patients 60 eases, Choose another 60 eases of healthy persons as normal controls, compared index difference between the groups. Result： The index of adiponectin, leptin and insulin resistance in the two groups was higher than normal controls, had significant difference, P〈0.05. Adiponectin in simple fatty liver subgroup had significant difference to steatohepatitis subgroup, P〈0.05. Adiponectin, leptin and insulinresistance have correlation between resistin and HOMA-IR, P〈0.05. Conclusion： Adiponectin, leptin and insulin resistance in non-alcoholic fatty liver disease had changes of different degree, Involved in non-alcoholic fatty liver disease incidence and development.
