摘 要:目的观察结直肠腺癌组织中细丝蛋白A(FLNa)、血管内皮生长因子(VEGF)的表达与微淋巴管密度(LMVD)的关系,并探讨其临床意义。方法应用免疫组化法检测46例结直肠腺癌组织及其癌旁正常结直肠组织中FLNa、VEGF和D2-40的表达水平,分析三者间及与临床病理参数的相关性。结果结直肠腺癌组织中FLNa阳性22例(47.83%)、VEGF阳性41例(89.13%),癌旁正常组织中分别为42例(91.30%)、15例(32.61%),两种组织中FLNa、VEGF阳性表达率比较均有显著差异(P=0.000);结直肠腺癌组织及其癌旁正常组织中的LMVD分别为(17.32±2.12)、(4.36±1.21)条,P=0.00;结直肠腺癌组织中FLNa、VEGF的表达及LMVD均与有无淋巴结转移、肝转移及癌灶的浸润深度有关,而与年龄、性别及癌灶的部位无关。结直肠腺癌组织中FLNa和VEGF的表达水平呈显著负相关(r=-0.445,P=0.000)。FLNa表达阳性者和阴性者的LMVD分别为(3.24±0.57)、(16.23±3.64)条,P=0.001;VEGF表达阳性者和阴性者的LMVD分别为(15.65±1.26)、(2.94±0.61)条,P=0.000。结论 FLNa、VEGF的表达水平和LMVD与结直肠腺癌的形成、发展密切相关,其机制可能为FLNa通过下调VEGF的表达水平抑制肿瘤微淋巴管生长,最终抑制肿瘤细胞经淋巴道扩散。Objective To observe the relationship between the expression of filamin A( FLNa), vascular endothelial growth factor(VEGF) and lymphatic microvessel density(LMVD) in coloreetal adenocarcinoma tissue, and to investigate their clinical significance. Methods The expression of FLNa, VEGF and D2-40 in colorectal adenocarcinoma tissues and colorectal normal tissues of 46 cases was examined by immunohistochemistry. The correlation between their expression and the clinicopathologic features was analyzed. Results The positive ease of FLNa and VEGF was 22 (47.83%) and 41 (89.13%) in eolorectal adenocarcinoma tissues, 42(91.30% ) and 15(32.61% ) in colorectal normal tissues, the positive rate of FLNa and VEGF expression in the two tissues showed significant difference(P = 0.000) ; The LMVD in cancer tissues and normal tissues was 17.32 ±2.12 and 4.36 ± 1.21 (P =0.00), respectively. Expression of FLNa, VEGF and LMVD value was correlated with lymph node metastasis, liver metastasis and the depth of tumor invasion, but not with the patient's age, gender, lesion site of tumor. The expression of FLNa was negatively related with the expression of VEGF in cancer tissues. The mean LMVD in FLNa positive cases and negative cases was 3.24 ± 0.57 and 16.23 ± 3.64 ( P = 0. 001 ) , respectively. The mean LMVD in VEGF positive cases and negative cases was 15.65 ± 1.26 and 2.94 ± 0.61 ( P = 0. 000), respectively. Conclusion During the occurrence and development of colorectal adenocarcinoma, there is a close relationship among the expression of FLNa, VEGF and lymphatic microvessel density of tumor. By suppressing the level of VEGF expression and the growth of lymphatic microvessel, FLNa gene may inhibit lymphatic metastasis.
