解剖学修复单侧唇裂术后继发人中嵴畸形的临床研究

摘　　要：目的探讨单侧唇裂术后继发唇畸形患者人中嵴平坦或凹陷畸形的手术修复方法。方法60例单侧唇裂术后继发畸形患者,随机分为两组(n=30),分别通过口轮匝肌垂直肌瓣和口轮匝肌榫卯型肌瓣矫正人中嵴畸形。根据术后患侧人中嵴高度、动态表现、手术瘢痕宽度以及患者的满意度,制定量化评分标准进行综合评价。结果60例患者伤口均一期愈合。术后随访6个月,口轮匝肌榫卯型肌瓣矫正人中嵴高度、患者满意度等均明显优于口轮匝肌垂直肌瓣组,差异有统计学意义(P<0.05)。结论根据口轮匝肌解剖学结构,利用榫卯型肌瓣修复唇裂术后继发人中嵴畸形有重要意义。Objective To explore the methods of flat or depressed deformity of philtrum column in patients with secondary deformities of unilateral cleft lip. Methods From 2005 July to 2012 June, 60 patients with secondary deformity of unilateral cleft lip were divided into 2 groups （n=30） and received the correction of philtrum column deformity with vertical orbicularis muscle flap and mortise-tenon joint orbieularis muscle flap. Height of phihrum column, dynamic performance, width of scar and patient satisfaction were measured to evaluate the surgical effects. Results All the patients were healed by first intension. The patients were followed up for 6 months. According to the scoring criteria, height of philtrmn column and patient satisfaction were statistically different between the two groups （P〈0.05）. Conclusion According to the anatomical structure of orbieularis muscle, mortise-tenon joint orbicularis muscle flap has important implications for the repair of phihrum column deformity.
