直肠癌LAR/ULAR术后预防性使用复方苯乙哌啶效果研究

摘　　要：目的探讨LAR/ULAR术后预防性使用复方苯乙哌啶的效果。方法收集2010年1月～2011年3月本中心收治的直肠癌行LAR/ULAR根治术181例患者的临床资料,根据术后是否预防性使用复方苯乙哌啶分成实验组和对照组,比较两组术后并发症发生率及术后早期恢复情况。结果实验组吻合口漏和肠梗阻发生率低于对照组(P<0.05或P<0.01),胃潴留、尿潴留的发生率两组间差异无统计学意义(P>0.05)。两组在术后住院天数、拔除胃管时间、拔除血浆引流管、拔除尿管、下床、饮水、进流食、术前及术后WBC、嗜酸粒细胞等方面差异无统计学意义(P>0.05)。结论在直肠癌LAR/ULAR根治术后,预防性使用复方苯乙哌啶是安全的,能降低吻合口漏及肠梗阻发生率。Objective To explore the effects of preventative use of diphenoxylate after low anterior resection （LAR） or ultra - low anterior resection （ULAR） on patients with rectal cancer. Methods The clinical data of 181 patients with rectal cancer who received LAR/ULAR from January 2010 to March 2011 were collected. According to the treatment of whether preventative diphenoxylate was used, they were divided into experimental group and control group. Comparison and analysis were made in the incidence of postoperative complications and early rehabihtations between the two groups. R^ults The incidence of stomal leak and intestinal obstruction in the experimental group was significantly lower than that in the control group （ P 〈 0. 05）. There was no significant difference in the incidence of gastric retention and urine retention （ P 〉 0.05 ）. There was no significant difference in postoperative hospital stay, the respective time of extracting gastric tube, plasma drainage tube, and urinary catheter, the time of first ambulation, drinking,and liquid diet, and the difference of WBC and eosinophils before and after the operation （ P 〉 0.05 ）. Conclusion The preventative use of diphenoxylate after LAR/ULAR on rectal cancer is safe and may ~ decrease the incidence of stomal leakage and intestinal obstruction.
