不同吸烟程度对牙龈上皮凋亡抑制蛋白表达的影响

摘　　要：目的观察不同程度吸烟患者的凋亡抑制蛋白Bcl-2的表达情况。方法选择37名行牙龈切除、拔除智齿及牙冠延长术的患者,年龄21～65岁,男34例,女3例。按照吸烟史分为Ⅰ组(吸烟史<6年,日均支数<11支)17例,Ⅱ组(吸烟史≥6年,日均支数≥11支)20例。选7例健康牙龈作为对照组。牙龈标本制作石蜡切片,行组织学观察。采用SP免疫组化法对凋亡抑制基因Bcl-2的表达进行组织定位。石蜡切片脱蜡、水化至水,按照SP免疫组化说明书步骤操作,常规脱水封片后行组织学观察。Bcl-2的阳性表达呈棕黄色,阴性为蓝色。PBS作阴性对照。结果 Bcl-2阳性表达主要分布在颗粒层和棘层,胞质呈棕黄或褐色,越接近基底层表达越强。吸烟组Bcl-2的在颗粒层和棘层的阳性表达细胞数较对照组减少且着色变浅,基底层细胞则都为阴性表达。凋亡阳性表达的细胞胞核呈棕黄褐色。对照组较吸烟组阳性表达弱,胞核着色轻。对照棘层和基底层的细胞凋亡阳性表达率分别为0.431和0.672,低于各吸烟组的平均水平。吸烟组牙龈上皮基底层及棘层细胞的阳性表达与对照组间具有显著性差异(P<0.05);吸烟Ⅰ组与吸烟Ⅱ组也存在显著性差异(P<0.05)。结论吸烟可导致牙龈Bcl-2基因的表达降低。Objective The observe the expression of inhibitor of apoptosis protein Bcl -2 in patients with different levels of smoking. Methods Thirty - seven patients aged 21 to 65 years who had underwent gingivectomy, removal of wisdom teeth or crown lengthening surgery were selected, of which 34 were males and 3 were females. According to the history of smoking, they were divided into smoking group I （ history of smoking 〈 6 years, the average daily consumption of cigarettes 〈 11 ） and smoking group H （history of smoking~〉6 years, the average daily consumption of cigarettes I〉 11 ） Seven cases of healthy gingivae were selected as the controls. Gingival specimens were made into paraffin sections for histological obser- vation. SP immunohistochemical method was used to detect the expression of Bcl - 2. According to the instruction of SP im- munohistochemistry, paraffin sections were dehydrated conventionally for histological observation after dewaxing and hydra- ting. It was yellowish - brown in the positive expression of Bcl - 2, and blue in the negative one. PBS buffer was consid- ered as negative control. Results The positive expression of Bcl - 2 was mainly distributed in the stratum spinosum and stratum granulosum, and it was yellowish - brown or brown in the cytoplasm. The closer it was to the basal layer, the stron- ger the expression was. In smoking groups, the positive expression of Bcl - 2 was weaker in the stratum spinosum and stra- tum granulosum than in the controls, and usually light coloring. The expression of Bcl - 2 was negative in the whole basal layer. The apoptotic cell nuclei with positive expression appeared brownish. Compared with smoking groups, the positive expression was weak in the controls and the nuclei were lightly stained. In the control group, the rate of positive expression was 0.431 and 0.672 respectively in stratum spinosum and stratum granulosum, which was significantly lower than that in smoking groups.
