贵阳市原发性肝癌危险因素的病例对照研究

摘　　要：目的:探讨贵阳市原发性肝癌主要危险因素。方法:采用成组病例对照研究,对3家医院的120例肝细胞癌患者和199例对照进行一般情况、饮酒史、吸烟史、既往疾病史、家族史、经济状况及其他生活习惯等调查;应用非条件logistic回归分析,通过比值比(OR)和95%可信区间(95%CI)进行评估,探讨与贵阳市原发性肝癌发生相关的主要危险因素。结果:5年前经济状况、肝癌家族史、吸烟、慢性肝病史、乙型肝炎病毒感染、饮酒在病例组和对照组间的差异有显著性,其OR值分别为0.324、5.185、2.649、2.554、10.961、3.345,95%CI分别为0.159～0.661、2.284～11.772、1.162～6.040、1.205～5.415、5.240～22.925、1.696～6.594。结论:肝癌家族史、吸烟、慢性肝病史、乙型肝炎病毒感染、饮酒可能是贵阳市肝癌发生的危险因素,5年前经济状况良好可能是其保护因素。Objective： To determine the main risk factors of primary hepatocellular carcinoma （PHC） in Guiyang city. Methods： Case-control. study was adopted to investigate 120 PHC patients and 199 controls by using a questionnaire to obtain information, including general situation, drinking history, smoking history, medical history, family medical history, economic conditions and other habits. The main related-factors were analyzed with non-conditional logistic regression model and evaluated by odds ratio （OR） and 95% confidence interval （95% CI）. Results： There are significant differences between PHC cases and controls in terms of economic status in past 5 years （ OR = 0.324）, fam- ily PHC history （ OR = 5. 185 ）, cigarette smoking （ OR = 2. 649 ）, chronic liver disease （ OR = 2. 554）, HBV infection （ OR = 10. 961 ）, and alcohol drinking （ OR = 3. 345 ）, and 95% CI were 0. 159 ～0. 661,2. 284 ～ 11. 772,1. 162 ～ 6. 040,1. 205 ～5.415,5. 240～22. 925,1. 696～ 6. 594 respectively. Conclusions： Family history of liver cancer, cigarette smoking, chronic liver disease, HBV infection, and alcohol drinking might be main risk factors of PHC in Guiyang city and better eco- nomic status in past 5 years is one of the protective factors for PHC patients.
