摘 要:目的:探讨预防性给予重组人红细胞生成素对雄性大鼠L5脊神经切断大鼠机械、热痛觉过敏的影响及可能的机制。方法:选用Sprague-Dawley(SD)雄性大鼠30只,选用L5脊神经切断神经病理性模型;随机分成3组(n=10):假手术+生理盐水组、损伤+生理盐水组、损伤+重组人促红细胞生成素(recombinant human erythropoietin,rhEPO)(5000 U/kg)组;术前1天给药,连续给药7天。采用von Frey纤维和Hargreaves法测定各组机械缩足反射阈值(mechanicalwithdrawal threshold,MWT)和热缩足潜伏期(thermal withdrawal latency,TWL);采用实时定量多聚酶链反应(real-time polymerase chain reaction,RT-PCR)的方法检测脊髓胶质细胞标志物白细胞分化抗原11b(cluster of differentiation antigen 11b,CD11b)和胶质细胞酸性蛋白(glial fibrillaryacidic protein,GFAP)的mRNA表达水平。结果:与生理盐水预给药手术组相比,预防性腹腔注射rhEPO 5000 U/kg,能显著缓解大鼠的机械、热痛高敏行为(P<0.05,P<0.01),降低脊髓胶质细胞白细胞分化抗原和胶质细胞酸性蛋白的mRNA表达水平(P<0.05)。结论:发现预防性给予rhEPO能预防神经病理性疼痛的发生。其效应与其抑制胶质细胞活性相关。Objective: To investigate the effect of rhEPO administrated intraperitoneally before or after nerve injury on the behavioral manifestation and the activation of glial cell in spinal cord using a rat model of neuropathic pain induced by L5 nerve transection. Methods: The thirty male Sprague-Dawley (SD) rats were randomly assigned into three groups: sham-operation with saline, L5 spinal nerve transection with saline and L5 spinal nerve transection with rhEPO (5000 U/kg ). rhEPO or saline was given intraperitoneally 1 day before surgery and continued daily to day 7 post-transection. The hind paw mechanical withdrawal threshold (MWT) and thermal withdrawal latency (TWL) of all animals were measured on day 7 after operation with von Frey hairs and Hargreaves tests, respectively. The transcription of mRNA of glial cell surface markers such as CDllb and GFAP was semi-quantitatively evaluated using real-time PCR assay. Results: Administration of rhEPO resulted in attenuation in mechanical and thermal hyperalgesia. Furthermore, rhEPO could markedly inhibit the level of mRNA expression of CDllb and GFAP in the spine cord (P 〈 0.05). Conclusion: We found the potential preventive value of rhEPO in the treatment of neuropathic pain, which correlated with the decreased spinal cord mRNA expression of CD1 lb and GFAP.
