摘  要:目的探讨经颈动脉灌注冷生理盐水联合体表降温对全脑缺血大鼠神经功能及炎症反应的影响。方法健康成年雄性SD大鼠96只,按照改良PULSINELLI四血管阻断法建立全脑缺血模型,实验分为脑缺血15min再灌注组(IR组)、脑缺血15min再灌注后全身亚低温组(H组)、脑缺血15min后经颈动脉联合体表降温组(CH组)、假手术组(S组)共4组,每组24只。分别于再灌注6、12、24、48h时每组随机抽取6只大鼠,进行神经功能缺陷评分(NDS),并采用ELISA法测定各组血清细胞间黏附分子-1(ICAM-1)和白细胞介素1β(IL-1β)的浓度。结果 CH组达亚低温时间较H组明显缩短(t′=-23.86,P<0.05)。与S组比较,IR组各个时间点神经功能评分低(F=78.59~191.90,q=11.61~31.80,P<0.01),H组和CH组较IR组评分高(q=4.32~1 1.61,P<0.01),CH组较H组各个时间点评分高(q=3.42~4.31,P<0.05)。与S组比较,其余各组各个时间点ICAM-1、IL-1β浓度明显升高(F=8.11~24.77,q=5.01~11.83,P<0.01);H组和CH组各个时间点ICAM-1、IL-1β浓度较IR组低(q=2.69~4.83,P<0.05);而CH组各个时间点与H组比较差异无显著性(P>0.05)。结论亚低温干预能明显减轻炎症反应,而经颈动脉联合体表降温方法具有更好的脑保护作用。Objective To investigate the effect of combined trans-earotid infusion of cold normal saline and surface coo- ling on neurological function and inflammatory response in rats with global brain ischemia (GBD. Methods A model of 15 min- GBI was created in 96 healthy, adult male Sprague Dawley rats according to modified PULSINELLI method. The experiment was equally divided into four groups: GBI group, GBI/reperfusion plus total body hypothermia group (H group), GBI plus combined trans-carotid and surface cooling (CH), and sham-operation (S) group. At reperfusion of 6,12, 24, and 48 hours, six rats from each group were randomly selected for grading their neurological deficit score (NDS), and detecting the levels of serum ICAM-1 and IL-1β by enzyme linked immunosorbent assay (ELISA). Results The time to reach mild hypothermia in CH group was much shorter than that in H group (t'=23.86,P〈0.05). Compared with S group, the NDS in IR group was lower at each reperfusion time point (F=78.59-193. 90;q=11.63-31.8;P〈0.01), that in H group and CH group was higher than that in IR (q =3.42-4. 31 ,P〈0.05). Compared with S group, the concentration of ICAM-1 and IL-113 was much higher than that in the other groups (F=8.11-24.77;q=5.01-11.83;P〈0.01), that in H and CH groups was lower than that in IR (q=2.69- 4.83,P〈0.05) ; but the differences of each time point between CH and H groups were not significant (P〉0.05). Conclusion Mild hypothermia intervention can obviously relieve inflammatory reaction, and the combined method has a preferable protection to the brain.
