骨肉瘤组织XIAP蛋白表达临床意义的研究

摘　　要：目的:观察骨肉瘤组织中X染色体连锁的凋亡抑制蛋白(XIAP)的表达,探讨其与骨肉瘤患者临床特征的关系,研究其临床意的义。方法:应用免疫组织化学方法检测50例骨肉瘤组织及8例瘤旁正常骨组织中XIAP的表达,分析其表达水平与患者临床特征相关性,并对骨肉瘤患者进行生存分期,研究XIAP表达水平与患者预后的相关性。结果:XIAP阳性表达主要定位于细胞质。XIAP在骨肉瘤组织中的阳性表达率为74%,明显高于瘤旁正常骨组织的25%,P=0.012。统计学分析显示,XIAP表达水平与患者的性别、年龄、肿瘤部位以及肿瘤大小无明显相关性,P>0.05;而与En-neking临床分期有关,Ⅲ期明显高于ⅡB期,P=0.009。生存分析显示,40例ⅡB期患者中高水平表达组中位无病生存时间短于低水平表达组,P=0.023;而所有患者中(40例ⅡB和10例Ⅲ期)高水平表达组中位总生存时间短于低水平表达组,P=0.004。结论:骨肉瘤组织XIAP高水平表达与预后不良相关,它对判断骨肉瘤患者的预后有一定的参考价值。OBJECTIVE： To illustrate the relationship between X-linked inhibitor of apoptosis protein （XIAP） and the clinical factors and prognosis of osteosarcoma. METHODS： The expression of XIAP in oteosarcoma or normal hone tissues which adjacent to osteosarcoma was detected by immunohistochemmistry. The related expression level of XIAP and clinical variables were calculated by Chi-square test. Kaplan-meier method was used to calculate the overall survival rate. RESULTS： The expression of XIAP was observed in cytoplasm. It was expressed both in osteosarcoma and normal bone tissues,but there was significant difference between the two groups （P = 0. 012）. The positive expression rate of XIAP was 74% in osteosarcoma tissues while only 25% in normal bone tissues. No correlations were found between the expres- sion of XIAP and the gender, age, size or location of tumor as well as clinical stage （P=0. 009）. Kaplan-meier analysis showed that in 40 cases of stage Ⅱ B patients ,the disease free survival time in XIAP-over-expression patients was lower than that in low-expression patients（P=0,023） ,and in 50 patients（40 cases of stage Ⅱ B and 10 cases of stage Ⅲ ） ,the overall survival time in XIAP-over-expression patients was also lower than that in low-expression patients （P = 0. 004）. CONCLUSION： XIAP is correlated with the prognosis of patients with osteosarcoma.
