摘　　要：目的:探讨LEEP在宫颈上皮内瘤变(CIN)临床应用的安全性及有效性。方法:回顾性分析45例CIN患者治疗的临床资料。结果:45例LEEP手术过程中,患者无特殊不适,手术时间平均10±5 min,术中出血平均15±5 mL。术后病理报告与术前诊断相比完全符合18例,一致率为40%;病理升级13例,比率为28.89%;病理降级14例,比率为31.11%。其中16例LEEP患者再次行筋膜外子宫全切术,术后病检均未见到CIN的残留。1年后随访发现,16例经LEEP及筋膜外子宫全切术后,复查无异常,达到治愈。29例经单纯LEEP治疗后,27例治愈,2例治疗无效,再次行LEEP手术及药物治疗好转。结论:LEEP治疗CIN,安全有效,价格实惠,手术时间短,患者疼痛小,出血少,并且能够保留完整、连续的标本进行病检,明显降低CIN的漏诊率,值得临床应用。Objective： To study the safety and efficacy of LEEP in treating cervical intraepithelial neoplasia （CIN）. Methods： The clinical data of 45 CIN patients were retrospectively analyzed. Results： 45 patients all had no special discomfort during the LEEP operation process. The operation time averaged （10± 5） min, and intraoperative bleeding averaged （15 ± 5） mL. The postoperative pathology reports of 18 cases were in complete conformity with their preoperative diagnosis, with a consistent rate of 40%; 13 cases had pathological upgrade, with a ratio of 28.89%; 14 cases had pathological downgrade, with a ratio of 31.11%. 16 patients with LEEP again had fascia outside total hysterectomy, and no residual CIN were found after the operation. One year later, we found the 16 patients who had LEEP and fascia outside total hysterectomy, had no abnormalities and were cured. Among the 29 patients with simple LEEP treatment, 27 patients were cured and 2 patients had once again LEEP surgery and drug treatment and then turned better. Conclusion： The LEEP is safe and effective treatment for CIN. It is cheap, with short operation time, less bleeding, and less pain. It can keep complete and continuous specimens from disease, and obviously decrease the CIN missed rate, thus it is worth of clinical application.
