摘 要:目的探索血清蛋白质组学诊断肝纤维化的价值。方法选取2008年8月至2011年1月上海中医药大学附属曙光医院收治的103例乙型肝炎后肝纤维化患者,同时选取28例慢性乙型肝炎患者和29名健康志愿者的血液,分离血清,以纳米磁珠预处理,MALDI-TOF-MS检测,应用Autoflex Analysis软件对采集图谱进行分析。结果筛选出15个差异表达的蛋白质峰,建立乙型肝炎肝纤维化血清蛋白质诊断模型;筛选出2个差异表达的蛋白质峰,建立慢性乙型肝炎血清蛋白质诊断模型;筛选出各组差异表达的蛋白质峰,建立乙型肝炎肝纤维化Child-PughA、B、C级血清蛋白质模型。结论以血清蛋白质组建立的乙型肝炎肝纤维化和慢性乙型肝炎诊断模型,有助于临床鉴别诊断肝纤维化。Objective To investigate the significance of serum proteomics for the diagnosis of cirrhosisMethods Serum samples were obtained from 103 patients with hepatitis B-associated cirrhosis ,29 with chronic hepatitis B (CHB) and 29 healthy individuals who visited Affiliated Shuguang Hospital of Shanghai University of Traditional Chinese Medicine, be- tween August 2008 and January 2011. Serum isolated from the specimens were treated with nano magnetic beads ( Bruker Daltonics GmbH) for subsequent assay of serum proteomics with matrix-assisted laser desorption ionization time of flight mass spectrometry (MALDI-TOF-MS, Bruker Daltonics GmbI-I) by using the ClinProTools 2. 1 software for data capturing and processing. Results Fifteen culminations with differential protein expression were identified for establishment of the serum proteomics diagnostic model of hepatitis B-associated cirrhosis, whilst 2 culminations were screened for establishing the chronic hepatitis B diagnostic model. Furthermore, the culminations with differential protein expression in each group were employed to establish the diagnostic model of cirrhosis with Child-Pugh stages A, B and C. Conclusion The estab- lished diagnostic models for hepatitis B-associated cirrhosis and chronic hepatitis B based on serum proteomics may be help- ful to differential diagnosis of cirrhosis.
