有晶体眼后房型人工晶体植入术治疗超高度近视的临床观察

摘　　要：目的:探讨有晶体眼后房型人工晶体植入术(ICL)治疗高度近视的有效性、安全性。方法:2010-03～2012-06行后房型有晶体眼人工晶体(ICL)植入术的高度近视患者18例(25眼),术前等效球镜度-14.75.0 D～-20.25 D,平均-17.52±1.73 D随访3月～24月,收集其术前术后裸眼及最佳矫正视力、等效球镜度数、眼压、角膜内皮细胞计数等资料并对结果进行统计学分析。结果:所有25只眼均成功植入ICL,术后裸眼视力均达到或超过术前最佳矫正视力。术前及术后3月等效球镜度(SE)、裸眼视力、角膜内皮细胞计数比较差异具有统计学意义(SE:t=-59.21,P=0.00;裸眼视力:t=-12.41,P=0.00;角膜内皮细胞计数:t=11.86,P=0.00),术前最佳矫正视力和术后3月裸眼视力比较差异无统计学意义(t=-0.84,P=0.41),术前、术后3月眼压比较差异无统计学意义(眼压:t=0.99,P=0.33)。有2例患者(2眼)术后轻度眩光。所有病例术眼观察期内均未发现自然晶体浑浊及继发性青光眼。结论:短期观察有晶体眼后房型人工晶体植入术保留了生理性调节,治疗高度近视疗效确切,具有良好的安全性。Objective ：To esitimate the security and effectiveness of posterior chamber （phakic） intraocular lens （ICL） implantation for the correction of high myopia. Methods：This study included twinty - five eyes of eighteen patients re- ceived ICL implantation from March 2010 to July 2012. The range of preoperative myopia was -14. 75.0 D to -20. 25 D,mean - 17. 52± 1.73 D. All of the patients had been followed up for 3 to 24months. The examinations in- eluded uncorrected visual acuity（ UCVA）, best corrected visual acuity （ BCVA ）, slit lamp examination, intraocular pres- sure, endothelial cell morphometry, etc. Results： All of 25 eyes were implanted ICL successfully. After operation, the un- corrected visual acuity（UCVA） of every eye was the same or even better than preoperative best - corrective visual acuity （BCVA）. The spherical equivalent（ SE）, uncorrected visual acuity（ UCVA）, corneal endothelium counting before and three months after the operation had significant difference （ SE ： t = - 59. 21, P = 0. 00 ; UCVA ： t = - 12.41,P = 0. 00 ; corneal endothelium counting：t = 11.86 .P = 0. 00）. No siginifieant differences were found between the best-corrective visual acuity preoperation and the uncorrected visual acuity three months after the operation （ UCVA ： t = - 0. 84,P =0. 41 ）. There was no siginificant differences inintraocular pressure before and three months after the operation （ t = 0. 99, P = 0. 33 ）. Dazzling sensation occurred in 2 cases （2 eyes）. No cataract occurred during the follow -up duration. Conclusions：Short- term observation shows that ICL implantation on the basis of preserving the natural lens appears to be an effective and safe method for high myopic.
