PathFileTM在老年患者根管治疗中的研究

摘　　要：目的:比较PathFileTM结合ProTaper,PathFileTM、HERO642、REVO-S三者联合,单独ProTaper、手用K锉在老年根管治疗中的应用效果。方法:选取老年因牙髓炎或根尖周炎需行根管治疗的患牙497例,用计算机产生随机数方法完全随机分为4组。PathFileTM结合ProTaper组(PP组)、PathFileTM、HERO642、REVO-S三者联合组(R组)、单独ProTaper组(P组)根管均采用冠向下法预备,大锥度牙胶尖冷侧压法充填,手用K锉组(k组)根管采用逐步后退法预备,2锥度冷牙胶尖侧压法充填治疗,前后拍摄根尖X线片评价根管预备、充填效果、预备时间及术后疼痛的发生率。结果:PP组和R组根管预备锥度、流畅度好,预备时间短,术后疼痛发生少且程度轻。PP组、P组、K组间根管预备效果、预备时间及术后疼痛有显著性差异(P<0.05);R组、P组、K组间根管预备效果、充填效果,预备时间及术后疼痛有显著性差异(P<0.05)PP组、R组和P组三组分别与K组间根管充填效果有显著性差异(P<0.05)。结论:应用PathFileTM进行根管前处理可以使老年患者根管预备成形更好、省时省力、安全性好。Objective： To compare clinical effects of PathFileTM combined with ProTaper, PathFileTM combined with HERO642 and REVO-S, ProTaper, hand K rasp in root canal therapy for the elderly patients. Methods： 497 teeth with pulpitis and periapicitis in elderly patients were selected and randomly divided into four groups. Root canal preparatory was performed by PathFileTM combined with ProTaper group （PP group）, PathFileTM combined with HERO642 and REVO-S group （R group）, ProTaper group （P group） with crown-down technique and hand K rasp group （K group） with step-back technique. Then the root canals were obturated with .04, .06 dental root-canal obturating points and lateral condensation technique in PP group, R group, P group, and with .02 dental root-canal obturating points and lateral condensation technique in K group. The efficiency of preparation and obturation were evaluated with radiographs before and after treatment. The operating time and the incidence of pain after treatment were recorded. Results： PP group and R group were showed to achieve continuously tapered root canal shape. The operating times of PP group and R group were short. The post-treament pain seldom occurred in PP group and R group. The efficiency of preparation, the operating time, the incidence of pain were significantly different between PP group, P group and K group （P 〈0.05）. The efficiency of preparation, the operating time, the incidence of pain were significantly different between R group, P group and K group （P 〈0.05）. There were significant differences of efficiency of obturation between PP group and K group, R group and K group, P group and K group （P 〈0.05）. Conclusions： PathFileTM showed good shaping ability and was safe for elderly patients in root canal treatment with high efficiency.
