细胞外基质金属蛋白酶诱导因子CD147和基质金属蛋白酶-9在胆囊癌及癌前病变组织中的表达

摘　　要：目的检测细胞外基质金属蛋白酶（MMP）诱导因子CDl47和MMP-9在胆囊癌及癌前病变组织中的表达，探讨两者与胆囊癌的发生、发展及预后的关系。方法应用免疫组织化学链霉菌抗生物素蛋白一过氧化物酶（sP）法检测CD147和MMP-9在42例胆囊癌、18例胆囊上皮中重度不典型增生、13例胆囊上皮轻度不典型增生、15例胆囊腺瘤及15例慢性胆囊炎组织中的表达。结果胆囊癌组织中CDl47和MMP-9的表达量（83．33％、88．10％）高于轻度不典型增生（38．46％、30．77％）、胆囊腺瘤（20．00％、26．67％）及慢性胆囊炎组织（6．67％、20．00％）（P〈0．01），与中重度不典型增生组织（61．11％、66．67％）比较差异无统计学意义（P〉0．05）。CDl47和MMP一9的阳性表达与胆囊癌的分化程度、临床分期、淋巴结转移明显相关（P〈0．05）。胆囊癌中CDl47和MMP-9的阳性表达呈正相关（rs=0．427，P〈0．01）。CDl47和MMP-9表达阴性的患者术后生存期高于表达阳性的患者。结论CDl47和MMP-9的表达在胆囊黏膜的癌变、癌细胞的浸润、转移及预后过程中起到重要作用。Objective To investigate the expression of extracellular matrix metalioproteinase in- ducer CD147 and matrix metalloproteinase-9 （MMP-9） in the gallbladder carcinoma and precancerous le- sions and their correlation with occurrence, progress, and prognosis of gallbladder carcinoma. Methods Streptavid-in-peroxidase （SP） immunohistochemistry was used to detect the expression of CD147 and MMP-9 in surgically resected specimens of 42 gallbladder carcinomas, 18 gallbladder epithelial atypical dysplasia Ⅱ-Ⅲ tissue samples, 13 gallbladder epithelial atypical dysplasia I tissue samples, 15 gallblad- der adcnomas and 15 chronic cholecystitis tissue samples. Results The positive rate of CD147 （83.33%） and MMP-9 （88.10%） proteins was higher in gallbladder carcinomas than in gallbladder epithelial atypical dysplasia I tissues （38.46% , 30. 77% ） , gallbladder adenomas （20. 00% , 26. 67% ） and chronic chole- cystitis tissues （6. 67% , 20.00% ） （P 〈0. 01 ） , but no significant difference was found in comparison to gallbladder epithelial atypical dysplasia Ⅱ - Ⅲ tissues （ 61.11%, 66. 67%, P 〉 0. 05 ）. The expression of CD147 and MMP-9 was positively correlated with the tumor differentiation, Nevin staging and lymph node metastasis （P 〈0.05 ）. The CD147 expression was positively correlated with MMP-9 expression in gall- bladder carcinoma （rs =0. 427 ,P 〈0. 01 ）. Patients negative for the expression of CD147 and MMP-9 had longer survival time than those with positive expression of CD147 and MMP-9. Conclusion CD147 and MMP-9 play important roles in carcinogenesis of gallbladder mucosa, and invasion, metastasis and progno- sis of gallbladder carcinoma.
