经阴道三维超声对子宫内膜癌肌层浸润的诊断价值探讨

摘　　要：目的探讨经阴道三维超声（3-DTVS）在子宫内膜癌肌层浸润的价值.方法收集2010年2月~2012年9月经诊刮确诊并行手术治疗的子宫内膜癌患者68例,术前行3-DTVS检查并测量病变部位体积,并对肌层浸润深度做出评估,与临床及病理指标行对比观察,进行回顾性分析.结果①68例患者,3-DTVS肌层浸润深度：无肌层浸润、浅肌层浸润、深肌层浸润者分别为8例、39例、21例.手术病理检查分期：Ⅰ,Ⅱ,Ⅲ,Ⅳ期分别为59例,6例,2例,1例.3-DTVS判断肿瘤浅肌层敏感性、特异性、阳性预测值、阴性预测值分别为94.29%,66.67%,91.67%,75.00%,深肌层分别为75.00%,84.62%,60.00%,91.67%.②3-DTVS的体积测量功能测量的浸润浅肌层、深肌层肿瘤的体积分别为（12.5±6.5）ml,（21.4±8.9）ml,差异有显著性.结论3-DTVS对判断子宫内膜癌肌层浸润有较高的价值,肿瘤体积可作为辅助判断肌层浸润的客观量化指标.Objective To examine the value of preoperative Three-dimensional Transvaginal Sonograph （3-DTVS） on endometrial carcinoma myometrial invasion （MI）.Methods A total of 68 women with endometrial carcinoma confirmed by postoperative pathology were retrospectively analyzed ,with the focus on the evaluation of the diagnosis of myometrial invasion, volume measurement and postoperative pathological staging by preoperative Three -dimensional Transvaginal Sonograph.Results ①Twenty-one patients were deep MI,39 superficial MI and 8 no myometrial involve-ment.The sensitivity,specificity,positive and negative predictive values of 3-DTVS in detecting superficial and deep MI were 94.29%,66.67%,91.67%,75.00% and 75.00%,84.62%,60.00%,91.67% respectively.②The tumor vol-ume of superficial and deep MI was（12.5 ±6.5）ml,（21.4 ±8.9）ml respectively.There was a significant difference be-tween the two groups.Conclusion Preoperative Three-dimensional Transvaginal Sonograph is helpful to the diagnosis of depth of myometrial invasion in endometrial carcinoma ,the measurement of tumor volume by 3-DTVS could be used as an assistant objective parameter.
