后腹腔镜加下腹部小切口治疗肾盂癌23例分析

摘　　要：目的评价后腹腔镜加下腹部小切口行肾盂癌根治术并总结经验。方法回顾性分析2008年7月～2012年7月施行的23例后腹腔镜加下腹部小切口行肾盂癌根治术的临床资料。结果手术均成功，无一例中转开放手术。平均手术时间（136．4±27．1）min，平均术中出血量（151．3±44．5）ml，平均术后肠功能恢复时间（1．8±0．3）d，术后引流管拔除时间2～5d，平均术后住院时间（7．2±1．0）d，术中术后未发生明显并发症。术后病理检查23例均为肾盂移行细胞癌，随访3～24个月，均未见肿瘤复发及转移。结论后腹腔镜加下腹部小切口行肾盂癌根治术具有手术时间短，创伤小、恢复快、术后并发症少、并不增加肿瘤种植的风险等优点，值得临床推广。Objectives To evaluate the retroperitoneal laparoscopy combined with the abdominal incision for radical nephroureterectomy and summarize our experience in the surgery. Methods From Jul 2008 to Jul 2012 ,the Clinical data were analysed 23 patients who underwent the retroperitoneal laparoscopy combined with the abdominal incision for radical nephroureterectomy. Results All operations were successful, there were no conversion to open surgery in this series. The mean operating time was （ 136.4 ±27.1 ） min, the mean blood loss was （ 151. 3 ±44.5 ） ml, the mean recovery of intestinal function was （ 1.8 ±0.3 ） d, the drainage tube was removed at 2 ~ 5 postoperative days, the mean duration of hospitalization ranged （7.2 ±1.0）d, no complications happen in hospital. Pathological findings after operation showed transitional cell carcinoma in all cases, follow up observations in the 23 cases for 3 - 24 months found no recurrence or metastasis. Conclusions The retroperitoneal laparoscopy com- bined the abdominal incision for radical nephroureterectomy has more advantages of shorter operative time, a smaller incision, a more rapid recovery , less Complications happen and minimal -invasive, It＇s a good choice for pelvis carcinoma.
