摘 要:目的探讨并比较翼状胬肉切除联合生物羊膜移植和翼状胬肉切除联合保存羊膜移植在治疗翼状胬肉中的治疗效果。方法对100例翼状胬肉患者,随机行翼状胬肉切除联合生物羊膜移植(n=57)或翼状胬肉切除联合保存羊膜移植(n=56),随访6个月,观察手术治疗效果。结果手术治疗后6个月,翼状胬肉切除联合生物羊膜移植组有6只眼(10.5%)复发,翼状胬肉切除联合保存羊膜移植组有5只眼(8.9%)复发。两组相比较,差异无统计学意义(P〉0.05)。翼状胬肉切除联合保存羊膜移植组患者术后恢复较快,异物感、不适感较轻微。所有患者无感染、排斥及其他并发症。结论翼状胬肉切除联合生物羊膜移植或翼状胬肉切除联合保存羊膜移植手术均能有效地治疗翼状胬肉,两者有着相似的复发率。翼状胬肉切除联合保存羊膜移植组患者术后相对恢复较快,自觉异物感、不适感较轻微。ObJective To compare the efficacy of pterygium resection combined with biotic amniotic membrane transplantation versus pterygium resection combined with preserved human amniotic membrane transplantation in the treatment of pterygium. Methods A total of 100 cases (113 eyes) were randomly assigned to receive pterygium resection combined with biotic amniotic membrane transplantation (n =57) or pterygium resection combined with preserved human amniotic membrane transplantation (n =56). Corneal wound healing, neovascularizafion, the presence or absence of conjunctival proliferation and hyperemia were analyzed at 6 months post-operatively. Results Six months after surgery, in the biotic amniotic membrane transplantation group, 6 eyes (10.5%) had recurrent pterygium, while in the preserved amniotic membrane transplantation group, 5 eyes showed recurrence (8.9%, P 〉0.05). However, patients in the preserved human amniotic membrane transplanta- tion recovered significantly faster compared with those in the biotic amniotic membrane transplanta- tion group. There were no significant complications associated with the use of biotic amniotic mem- brane transplantation or preserved human amniotic membrane including infection and rejection. Con- clusions Both of the biotic amniotic membrane transplantation and preserved human amniotic mem- brane transplantation are effective in the treatment of pterygium. They have similar recurrence rates, but patients undergoing preserved human amniotic membrane transplantation experience faster recov- ery compared with undergoing biotic arnniotic membrane transplantation.
