摘  要:目的:研究乏氧诱导因子-1α(HIF-1α)基因及糖酵解相关基因己糖激酶-Ⅱ(HK-Ⅱ)、乳酸脱氢酶-Ⅴ(LDH-Ⅴ)在原发性肝细胞癌(HCC)组织中的表达状态,并阐释三者对HCC生物学行为的影响。方法:收集行手术治疗的56例新鲜肝癌组织标本,通过免疫组织化学技术检测HIF-1α、HK-Ⅱ和LDH-Ⅴ等基因在肝癌组织中的蛋白表达情况,并分析其与HCC临床病理学指标之间的关系。结果:在肝细胞癌组织中HIF-1α、HK-Ⅱ和LDH-Ⅴ蛋白的阳性表达率分别为71.4%、75.0%和67.9%。HIF-1α、HK-Ⅱ及LDH-Ⅴ三者的阳性表达均与HCC的有无肝硬化背景、肿瘤细胞的分化程度、临床分期、有无肝门淋巴结转移、有无门脉/胆道癌栓的形成、有无静脉侵袭及包膜完整与否等因素相关,P<0.05;而与HCC患者的性别、年龄、HBsAg阳性或阴性、血清AFP高低及癌灶的直径大小等指标无关,P>0.05。结论:HK-Ⅱ、LDH-Ⅴ基因与HIF-1α基因的同步表达在HCC的发生、发展中可能发挥协调作用,共同促进肝癌细胞的侵袭与转移,反映了HCC的生物学行为,可以将三者作为预测HCC预后的重要生物学指标。OBJECTIVE: To investigate the expression of HIF-1α, HK-Ⅱ, and LDH-Ⅴ genes in hepatocellular carcinoma (HCC) tissues,and analyze their influences on biological behaviors of HCC. METHODS: Tumorous tissue specimens were obtained randomly from 56 patients with HCC who underwent partial hepatectomy. The expressions of HIF-1α, HK-Ⅱ and LDH-Ⅴ genes protein were detected with IHC technique and the relationships with clinieopathological eharacteristi parameters of the HCC were analyzed. RESULTS: The positive expression rate of the HIF-1α, HK-Ⅱ and LDH-Ⅴ genes protein in HCC tissues was 71.4%, 75.0 %and 67. 9%, respectively. Overexpressions of these proteins were closely associated with the elinicopathological characteristic parameters such as the background of liver cirrhosis,differentiation degree of cancer cells,clinical stage,lymph node metastasis of hepatic hilum,the existence of portal vein or biliary tract tumor emboli,microscopic venous invasion,and capsule formation (P〈0.05) ,but not correlated to the sex and age of the HCC patients, the status of HBsAg, the value of blood serum AFP and the size of primary tumors (P〈0. 05). CONCLUSIONS: In the procedure of canceration of liver cells and tumor growth,the HIF-1α, HK-Ⅱ and LDHV genes might play crucial roles,which might collectively participate in progression of HCC. The synchronous overexpressions of the HIF-1α, HK-Ⅱ and LDH-Ⅴ genes can reflect the biological behaviors of HCC,and they are three important indexes for HCC invasion and metastasis and could serve as novel biological markers of predictor in evaluating the prognosis of patients with HCC.
