摘 要:采用免疫组化 L SAB法及图像分析技术 ,定量检测 41例细支气管肺泡癌 (BAC)组织中的神经元特异性烯醇化酶 (NSE)和 α1 -抗胰蛋白酶 (α1 - AT)的表达 ,探讨其与 BAC的组织发生及其生物学行为的关系。结果表明 ,NSE和 α1 - AT阳性率分别为 70 .7% (2 9/ 41)和 85 .4% (35 / 41) ,α1 - AT的表达的强弱与组织亚型有关 ,肺泡型表达强度高于其他三型 (P<0 .0 5 ) ,乳头状型高于粘液型和硬化型 (P<0 .0 5 ) ,粘液型和硬化型间无明显差异 (P>0 .0 5 ) ,NSE表达的强弱与组织亚型无关 (P>0 .0 5 ) ;淋巴结转移组 NSE表达的强度 (0 .10± 0 .0 2 )明显高于无转移组 (0 .0 7±0 .0 2 ) ,淋巴结转移组 α1 - AT表达的强度 (0 .13± 0 .0 2 )则明显低于无转移组 (0 .15± 0 .0 2 ) ,差异均有显著性 (P<0 .0 1)。提示 BAC也具有产生 α1 - AT的能力并大部分伴神经内分泌分化 ,NSE和 α1 - AT表达的强弱与By using immunohistochemistry LSAB method and imaging analysis technique, the expression of NSE and α 1 antitrypsinase (α 1 AT) in 41 cases of bronchioalveolar carcinoma (BAC) was quantitatively detected to explore the relationship between α 1 AT and NSE expression in BAC tissues and histogenesis and biobehaviors. The results showed that the positive rate of NSE and α 1 AT was 70.7 % (29/41) and 85.4 % (35/41). The expression of α 1 AT was related with histological subtypes: the expression level of α 1 AT in alveolar BAC higher than in the other three types ( P <0.05), in papillary BAC higher than in mucinous BAC or sclerosing BAC ( P <0.05). No significant difference in α1 AT expression was found between mucinous BAC and sclerosing BAC ( P >0.05). The expression of NSE was not related with the subtypes ( P >0.05). The expression level of NSE in the patients with lymph node metastasis (0.10±0.02) was obviously higher than those without lymph node etastasis (0.07±0.02). However, the expression level of α 1 AT in the group of lymph node metastasis (0.13±0.12) was significantly lower than in the group without lymph node metastasis (0 15±0 02) (all P <0 01). It was suggested that BAC was capable of producing α 1 AT and most of them were differentiated neuroendocrinologically. The expression levels of NSE and α 1 AT were closely related with the biobehaviors of BAC.
