摘 要:目的 探讨缺血预处理 (Ischemic Proconditioning IPC)对未成熟心脏全心缺血再灌注损伤的影响。方法 对经历 5 Min缺血、10 Min再灌注的幼兔 (14~ 2 1天 )心脏进行离体灌注 ,观察其在生理温度 (39℃ )下接受 30 Min缺血、40 Min再灌注的心肌酶释放、心肌能量及病理变化。结果 IPC组全心缺血后心脏停跳持续时间明显延长 (P<0 .0 1) ,心肌 ATP含量明显减少 (P<0 .0 0 1) ;光、电镜显示两组心肌损伤差别不显著。结论 单纯缺血预处理并不能保护遭受全心缺血再灌注损伤的未成熟兔离体心脏。Objective Protection by preconditioning(IPC) in adult hearts of all species has been extensively studied and reported.In this study,we investigated whether immature hearts could be preconditioned and probed the effect of IPC on immature myocardium.Methods Aerobically perfused in Langendorff mode at 39℃,isolated immature rabbit hearts(14~21 days old) were subjected to an IPC stimulus consisting of 5 minutes of global ischemia and 40 minutes of reperfusion.This was followed by 30 minutes of global ischemia and 40 minutes of reperfusion.The changes in energetic metabolism and morphology were determined.Results The IPC group had much longer cardiac arrest time (16.23±1.74min versus 13.50±1.82min in controls,P<0.01).The CK-MB leakage in IPC group was increased,although the result was not significantly different from the control.The myocardial level of ATP in preconditioning group at the end of the reperfusion was significantly decreased(123.85±17.42μg/g versus 167.21±16.53μg/g in controls, P <0.001).Light and electron microscopic findings of myocardium showed the preconditioned hearts were similarly damaged.Conclusion Single IPC can not protect the immature rabbit hearts experiencing global ischemia and reperfusion in vitro.Moreover it could result in damage to the myocardium;the reason for this may be associated with longer-lasting cardiac arrest time and more consumption of energy.
