事件相关电位P300在甲基苯丙胺依赖者认知能力检测中的应用

摘　　要：目的探讨甲基苯丙胺依赖者认知功能状况及事件相关电位P300特征。方法随机将年龄在18～65岁之间符合CCMD-3甲基苯丙胺依赖者诊断标准患者30例作为实验组,选择30例健康者作为对照组。各组分别予P300检测和洛文斯顿认知评定量表筛查实验,将两组结果加以分析、比较。结果①与对照组相比,实验组Cz、Pz点潜伏期明显延长(P<0.05或0.01);实验组Cz、Pz点P2、P3波幅明显延长(P<0.05或0.01);②实验组洛文斯顿认知评定量表7个因子分及总分值均低于对照组,差异有统计学意义(P<0.05或0.01);③Pz点潜伏期及波幅与洛文斯顿认知评定量表中7个因子分具有相关性:为Pz点潜伏期与各因子分及总分呈负相关(r=-0.348～-0.527;P<0.05或0.01);Pz点波幅则与各因子分及总分呈正相关(r=0.337～0.523;P<0.05或0.01)。结论甲基苯丙胺依赖者存在认知功能障碍,P300可在临床中用于评价甲基苯丙胺依赖者认知状况。Objective To explore the cognitive function and event related potential P300 characteristic of patients with metham- phetamine dependence. Methods A total of 30 patients（among 18-65 years old）met with CCMD--3 for Methamphetamine De- pendence Patient criteria were recruited as research group. 30 matched sexual and age healthy adolescents were selected as normal control group. Loewenstein Occupational Therapy Cognitive Sssessment （LOTCA）was used to investigate the cognitive ability of the subjects. P300 were recorded with all the subjects. A comparison was made between patients and healthy subjects. Results The latencies of Cz and Pz were significantly longer in patients with methamphetamine dependence than those in normal controls, the amplitudes of Cz and Pz were significantly lower in patients（P〈0.05 or〈0. 01）. The seven factors scores and the total score of LOTCA test in patients with methamphetamine dependence were significantly lower than those in normal controls（P〈0. 05 or〈 0.01）. There was significant correlation between the latency and seven factors scores of L OTCA as well as between the amplitude of P300 and seven factors scores of LOTCA：The latency of Pz was negatively correlated with factors scores and amplitude of Pz was positively correlated with LOTCA scores（P〈0.05 or〈0.01）. Conclusion Patients with methamphetamine dependence pre- sent impaired cognitive function. P300 can be used as a clinical index for appraising the cognition ability of patients with metham- phetamine dependence.
